The compensatory stage of renal function is a period of chronic renal failure. During this period, the glomerular filtration rate in a normal person is about 120 ml per minute. During this period, people's glomerular filtration rate will gradually decrease by 0-60 ml/min, and the number of renal units will decrease by about 20%-25%. Therefore, there are actually no special clinical manifestations. Blood muscle enzymes and blood urea nitrogen are usually normal or sometimes slightly elevated.

Azotemia stage: During this stage, the glomerular filtration rate is reduced to 25 ml/min, the number of nephrons is reduced by 50%-70%, the renal concentrating function is impaired, nocturia or polyuria occurs, anemia of varying degrees, azotemia is often present, and blood creatinine and urea nitrogen are increased. Clinically, symptoms may include fatigue, loss of appetite, nausea, and mild general discomfort. During this period, if renal function protection is neglected or the body is subjected to additional loads, such as severe vomiting, diarrhea, hypovolemia, severe infection, and the use of nephrotoxic drugs, renal function may rapidly deteriorate and even fail.

Renal failure stage (pre-uremia): When the glomerular filtration rate decreases to 10-15 ml per minute, the number of renal units decreases by about 70%-90%, the renal function is severely damaged, and the body's metabolism, water, electrolyte, and acid-base balance cannot be maintained. It is impossible to maintain a stable internal environment of the body, resulting in a significant increase in blood creatinine and urea nitrogen, urine concentration and dilution dysfunction, acidosis, water and sodium retention, low calcium, high phosphorus, high potassium and other imbalances. There may be obvious anemia and gastrointestinal symptoms, such as nausea, vomiting, and decreased appetite. There may also be neuropsychiatric symptoms, such as fatigue, inattention, and lack of energy.
