As the disease progresses, the papules gradually merge into flakes, forming a heroic lichenified change. At this time, the affected skin is dry, thickened, and the skin lines are deepened, intertwined, with raised skin ridges in a diamond or polygonal shape with clear boundaries. I have seen this disease in young and middle-aged men.

Syndrome differentiation

This disease can be clinically divided into three types. The initial onset is mostly due to wind-heat stagnation, followed by blood heat and wind excess, and over time it leads to blood deficiency and wind dryness.

Wind-heat invades the skin surface: there is only paroxysmal itching locally, mostly due to stimulation such as scratching or friction. A lichenoid rash appears on the local skin, which is polygonal (or round), as big as a hat head, with light red or normal skin color. There are also symptoms such as depression, insomnia and irritability.

Blood deficiency and wind dryness, evil pathogens accumulate on the skin surface: papules fuse into patches, the skin becomes thickened, dry and rough, or there are a small amount of grayish white scales, which become lichenification, and paroxysmal itching intensifies, often resulting in scratches, blood scabs and secondary infections due to itching and scratching.

Wind-heat stagnation: more common in localized patients, with skin lesions in patches, mainly papules, light red or light brown, rough and thickened, with paroxysmal severe itching, and quickly turning into lichenification after scratching. The tongue coating is thin or slightly greasy, and the pulse is slippery or slow.

Heat in the blood and excess wind: more common in patients with generalized disease. The skin lesions are red and spread all over the body. They appear as large infiltrative erythematous patches with scratches, blood scabs, and lichenification. The itching is constant, especially at night, accompanied by restlessness, thirst, insomnia, dreaminess, impatience and irritability. The tongue is red, the tongue coating is thin, and the pulse is slippery, rapid or stringy.

Blood deficiency and wind dryness: Skin lesions do not heal after a long illness and become increasingly severe. Local lesions become thicker and rougher, lighter or light brown in color, dry and scaly on the surface, and it itches severely, especially at night. The tongue is pale, the coating is thin, and the pulse is thin.
