In the early or acute stage, the affected area has patches of erythema, dense or scattered small papules, or blisters that are difficult to see with the naked eye. In severe cases, there are large areas of exudate and erosion. In the subacute state, the exudate decreases and scabs form, and the affected area turns from bright red to dark red, without large areas of erosion. In the chronic state, the exudate is less or completely dry and scabs form, which are often mixed with scales to form scaly scabs. The affected area is darker in color or pigmentation occurs, and sometimes the pigmentation is reduced. Cracks are prone to occur in the skin lines, especially in areas with greater movement. Long-term friction and scratching can cause significant lichenification, which is difficult to distinguish from neurodermatitis (chronic simple lichen).

Eczema often takes many forms, is easily relieved, aggravated or recurred, and the boundaries are generally not clear. The rash tends to occur on both sides and is more or less symmetrical. Depending on the acuteness or chronicity of the disease, it may present with erythema, papules, blisters, erosions, scales, crusts, increased or decreased pigmentation, chapped or lichenified skin. Several of these manifestations are often mixed together and sometimes occur one after the other. If there is secondary infection, there may also be skin lesions such as pustules.
