Other areas of the foot that may be affected are the dorsum of the foot, the heel, and the ankle. In addition to affecting the joints, uric acid crystals can also be deposited under the skin in areas called tophi. During the acute attack of stroke, symptoms should be relieved by resting, raising the affected limb, wearing shoes with hard soles, and open-toed postoperative shoes. Medical treatment includes high-dose nonsteroidal anti-inflammatory drugs or colchicine. When chronic gouty nodules are present, the symptomatic areas can be cleaned or the deposits removed. Severe joint destruction due to gout and pyrophosphate arthropathy is treated the same as degenerative arthritis of the first metatarsophalangeal joint or hallux rigidus. Joint debridement, implant arthroplasty and arthrodesis are all reasonable options for treating moderate to severe joint destruction.
