One theory is that the blood-soaked hemostatic plug serves as a culture medium for rapid bacterial growth. Hemostatic plugs are usually made of polyester foam, which is a more likely culture medium for bacteria than cotton and rayon.

During the use of menstrual sponges, diaphragms, and cervical caps, these materials are either left in the vagina for a long time - more than 30 hours, or sponge fragments from the sponge pad are left in the vagina. Both situations can provide a good environment for bacterial growth.

The route by which bacterial toxins enter the bloodstream may be related to the use of vaginal hemostatic plugs. The researchers found that the hemostatic plug slipped into the vagina, causing micro-tears in the vaginal wall and piercing the blood vessel wall. In particular, the use of hemostatic plugs with high absorption properties, if left in place for too long or used when the menstrual flow is small, can cause the vagina to become too dry and easily cause vaginal wall tears.

Researchers have ruled out several other causes of toxic shock. Women should spray and bathe with deodorant; underwear and other clothing will not work. In addition, the patient's menstrual history, drug and alcohol use, smoking, swimming or bathing, or sexual intercourse were not associated with toxic shock syndrome.
