Treatment of perianal eczema: local antipruritic measures are the primary measure

Perianal eczema is divided into acute eczema and chronic eczema. Symptomatic treatment is the key. At present, local antipruritic measures are the primary measure. You can prepare 4% boric acid, 0.1% levulinol and 2-3L of water, and then use it to scrub the affected area, which has a good effect.

(I) Treatment of acute perianal eczema

1. Local antipruritic measures are the primary measure to cut off the vicious cycle of itching-scratching-itching. The effective and quick method is wet hot compress (foam pack) or wet cold compress. The specific method is: prepare 2-3L aqueous solution of 4% boric acid and 0.1% levulinol, the water temperature is about 45°C, put it in a container, and use absorbent cotton to make a cotton pad, the area is slightly larger than the lesion surface, 1-2cm, and 3-4cm thick. Soak the cotton pad in the above-mentioned liquid, take it out and wring it dry, apply it on the surface of the eczema lesion without dripping, make sure that the pad is in close contact with the lesion skin, and then apply plastic film on the outside of the pad, and hang it between the buttocks with a boric belt, which can immediately stop itching. Apply 3-4 times a day, each time for 1-1.5 hours, and keep the water temperature at about 45°C. The cold compress method is the same, the solution temperature should be 5-10℃. In summer, ice cubes can be added to the solution to lower the water temperature.

2. After the above treatment, when the itching is significantly relieved and the serous exudation is suppressed, you can use dry mixed shock lotion externally, such as white lotion, calamine lotion, etc. If necessary, add an appropriate amount of corticosteroid powder to the lotion, but the concentration should not be too high.

3. Regularly and quantitatively administer intraocular hormones, anti-inflammatory mediators, antibiotics, vitamins, sedatives, etc. according to the patient's general condition.

(II) Treatment and prevention of recurrence of chronic perianal eczema

1. Treatment

⑴. For hypertrophic skin lesions, a higher concentration of corticosteroid ointment is used for encapsulation treatment. The composition of the solution used is as follows:

Dexamethasone 0.1-0.3 g

Absolute alcohol 2.0ml

Dimethyl sulfoxide 40.0g

Glycerin 20.0ml

95% alcohol added to 100.0ml

Usage: Clean the anus with warm water and soap, wipe dry, apply the above ointment, apply 2 to 3 times in a row, apply ordinary ointment or Fuji Ning ointment on it after it dries. The sealing can last for 6 to 10 hours and should be changed once a day. It is best to apply the treatment before going to bed and remove the sealing the next morning.

⑵. For milder skin lesions, apply various ointments, creams or lotions containing corticosteroids.

2. Prevention: Anal eczema is prone to recurrence after recovery. Finding and avoiding the inducing factors requires cooperation between doctors and patients. Anal eczema is often directly related to eating irritating foods, which should be prohibited or avoided.
