Many people may not have heard of Crohn's disease, which is also an esophageal digestive disease. Most patients have an unhygienic daily diet. Or it may be a disease caused by prolonged weakness of the spleen and stomach, which results in stagnation of liver qi. How should Chinese medicine treat it? Crohn's disease is mostly caused by improper diet, invasion of external pathogens, emotional distress, and long-term illness and physical weakness. Internal dampness, stagnation of Qi and blood, and spleen and kidney deficiency are the key pathogenesis of Crohn's disease. It has common characteristics of underlying deficiency and superficial excess, and a mixture of deficiency and excess. The underlying deficiency is caused by spleen and kidney Qi deficiency or Yang deficiency, while the superficial excess is caused by damp-heat stagnation, liver Qi stagnation, or Qi stagnation and blood stasis. Syndrome differentiation and treatment: 1. Syndrome of internal accumulation of damp-heat: abdominal pain that resists pressure, urgent diarrhea, or loose and uncomfortable stools, yellow-brown and smelly stools, or red and white dysentery, or constipation, thick pus around the anus, distending pain and burning sensation in the anus, thirst and preference for cold drinks, short and yellow urine, red tongue with yellow and greasy coating, and stringy or slippery and rapid pulse. Treatment should be aimed at clearing away heat and dampness, regulating qi and promoting blood circulation. The prescription is Baitouweng Decoction, which contains 15g Baitouweng, 12g Huangbai, 6g Huanglian, and 12g Qinpi. For those with excessive heat and toxicity, add Forsythia suspensa, Taraxacum mongolicum, Rehmannia glutinosa, and Cortex Moutan to clear away heat, cool blood, and stop bleeding; for those with severe bloody stools and a lot of mucus, add Atractylodes lancea and Coix seed; for severe abdominal pain, add Corydalis yanhusuo, Linderae scutellariae, and Citrus aurantium to regulate qi and relieve pain; for hard lumps in the abdomen, add Trillium and Curcuma zedoaria; for severe body heat, add Pueraria root. 2. Syndrome of cold-dampness obstructing the spleen: acute and violent abdominal pain, which is relieved by warmth; loose and thin stools, or clear and watery stools; or red, white, sticky and frozen diarrhea, with more white and less red; feeling of heaviness in the head and body; pale tongue with white greasy coating; and weak and soft pulse. Treatment should be aimed at removing dampness and dispersing cold, regulating qi and warming the middle. The prescription is Wei Ling Decoction, which includes 10g of Atractylodes lancea, 10g of Magnolia bark, 5g of Tangerine peel, 5g of Licorice root, 10g of Atractylodes macrocephala, 5g of Cassia twig, 10g of Poria cocos, 10g of Alisma orientalis, 10g of Ginger, and 10g of Red Dates. For those who suffer from abdominal pain and are afraid of cold and prefer warmth, add roasted ginger to warm the middle and dispel the cold; for diarrhea with red, white, sticky and frozen fruits, with more white and less red, remove Alisma orientalis and Poria cocos and add Paeonia lactiflora and Angelica sinensis to activate blood circulation and nourish the body; for those who suffer from long-term diarrhea, add Coix seed, Chinese yam, red ochre, pomegranate peel, black plum and Terminalia chebula to strengthen the spleen, eliminate dampness, and astringe the intestines to stop diarrhea. 3. Spleen and Kidney Yang Deficiency Syndrome: The disease course is long, with dull abdominal pain that comes and goes, the patient prefers warmth and pressure when in pain, thin pus around the anus, dull pain in the anus, loose stools, or diarrhea at dawn, loss of appetite, fatigue, cold limbs, back pain and polyuria, pale tongue, or fat tongue with red marks, white tongue coating, deep or thin and weak pulse. The treatment is to strengthen the spleen and warm the kidney, consolidate and stop diarrhea. The prescription is Shenling Baizhu Powder combined with Sishen Pills, including 9g lotus seed, 9g coix seed, 6g Amomum villosum, 6g Platycodon grandiflorum, 12g white hyacinth bean, 15g white Poria cocos, 15g ginseng, 9g licorice, 15g Atractylodes macrocephala, 15g yam, 6g nutmeg, 12g Psoralea corylifolia, 6g Schisandra chinensis, and 6g Evodia rutaecarpa. For severe abdominal pain, add white peony root to relieve the pain; for lower abdominal distension, add Atractylodes lancea, fennel, and Citrus aurantium to regulate qi and eliminate fullness; for loss of appetite, add hawthorn, Shenqu, malt, etc.; for those with severe deficiency and cold, and watery diarrhea, add Lizhong Decoction with aconite and cinnamon; for loose stools, add red ochre and Terminalia chebula to astringe the intestines and stop diarrhea. |
