Primary factors:

The primary lesions of hip osteoarthritis are degeneration, chondromalacia, erosion, etc. of hyaline cartilage, followed by changes in the synovium, joint capsule and muscles.

Secondary factors:

The causes of secondary osteoarthritis include: congenital abnormalities of the joints, trauma, acquired unevenness of the joint surfaces, malalignment caused by extra-articular deformities, and joint instability.

Iatrogenic factors:

Iatrogenic factors such as long-term inappropriate use of corticosteroids.

Clinical manifestations

The main clinical symptoms are joint pain and inflexible joint movement. Physical examination showed swollen joints, fluid in the joints, and muscle atrophy; there were creaking sounds during active and passive joint movements, and there were varying degrees of limited movement and muscle spasms. Some elderly people also have thickening of the distal interphalangeal joints. Hip arthritis is manifested by pain in the outer buttocks, groin and other areas, which may radiate to the knee. Specifically, there are hip pain, swelling, internal fluid accumulation, cartilage wear, bone spur growth, joint deformity, limited internal rotation and extension of the hip, inability to walk, and even bedriddenness. The incidence of hip osteoarthritis in the Chinese population is lower than that in the Caucasian population.

Routine blood and urine examinations and erythrocyte sedimentation rate, mucin, rheumatoid factor, etc. of patients with this disease are all within normal range. The color, transparency and mucin clot test of synovial fluid are normal, the white blood cell count is between 200 and 2000/mm3, and there are no bacteria or crystals under microscopic examination, but cartilage fragments and fibers can be seen. The degree of cartilage degeneration can be roughly estimated from the number of fragments.
