Should ear granulation be removed?

Many friends actually have granulation tissue on their ears. In fact, in most cases, granulation tissue will not cause pain or other discomfort to the patient, so you can ignore it. However, if the granulation tissue continues to swell or become painful, the patient must seek timely treatment to avoid hearing loss due to missing the best treatment time.

Patients should undergo modified radical mastoidectomy or radical mastoidectomy as soon as possible to completely remove the lesions and prevent complications in order to obtain a dry ear, and tympanoplasty should be performed as appropriate to improve hearing. The purpose of radical mastoidectomy is to:

① Thoroughly remove cholesteatoma , granulation tissue, polyps, and diseased bones and mucosa in the tympanic cavity, tympanic sinus, and mastoid cavity.

②Reconstruct hearing. During the operation, the middle ear structures that are closely related to the sound transmission function are preserved as much as possible, such as the auditory ossicles, residual tympanic membrane, Eustachian tube mucosa, and even the complete external auditory canal and tympanic groove, and on this basis, hearing is reconstructed in the first or second stage.

③ Strive to dry the ears.

Chronic suppurative otitis media is divided into three types, and cholesteatoma otitis media belongs to the third type, which is also the most serious of the three types and is prone to complications. The so-called cholesteatoma is the accumulation of shed epithelium in the middle ear. It grows larger and larger and continues to expand around, thus destroying the adjacent bone. There are important blood vessels and nerves upward, inward, and backward. Once the nerves are damaged, it will cause facial nerve paralysis. If it expands into the skull, it will form life-threatening complications such as brain abscesses. Therefore, once cholesteatoma otitis media is diagnosed, surgical treatment is necessary. The main purpose of surgery for this type of otitis media is not to improve hearing or to stop pus discharge, but to prevent complications. Of course, during the operation, the doctor will do his best to help improve the postoperative hearing and dry ear rate. However, due to the nature of the disease, the dry ear rate after surgery for cholesteatoma media is not very high, only about 80%. Generally speaking, if the patient's condition meets the requirements for tympanoplasty, tympanoplasty can be performed during the operation, and hearing will be improved after the operation. If there are no conditions for tympanoplasty and only radical cholesteatoma removal is performed, hearing will not decrease after the operation.

As for the pus discharge now, it means that the infection has not been controlled, which means that the surgery has not achieved the purpose of drying the ear. Change the dressing regularly, clean the surgical cavity, perform bacterial culture and drug sensitivity test on the pus, and select extremely sensitive antibiotics for treatment based on the results of the drug sensitivity test.

When granulation tissue grows in the ear, you should also observe whether the ear has other symptoms of discomfort, such as pus discharge, hearing loss, etc., to prevent the disease from affecting hearing. When the patient is treating the disease, you must also pay attention to ear cleaning and care. Do not let water enter the ears when the patient washes his hair or takes a bath to prevent water from flowing into the ears and forming an infection, which will aggravate the patient's disease and have a great impact on the treatment of the disease.
