2. Technical Principle

Maxillary osteotomy and advancement surgery is mainly used to treat crossbite caused by maxillary retrusion. The operation is performed by making an incision in the mouth. After peeling off layer by layer and exposing the bone tissue, the osteotomy instrument is used to cut the bone and move the maxillary bone as a whole backward to correct the maxillary retrusion, thereby achieving the purpose of correcting the crossbite.

1) Suitable population: Those with simple maxillary retrusion or combined maxillary retrusion and mandibular protrusion, preferably after 16-18 years old.

2) People who should not eat

① Those with heart disease, hypertension, diabetes or other important organ diseases.

② Those with severe scarring constitution.

③ Those with odontogenic or parotid infection foci in the maxillofacial region.

④ Women during pregnancy or menstruation.

⑤ People with mental disorders.

4. Preoperative planning: Preliminarily determine the bone tissue that should be retracted and mark it .

5. Technical methods

① The patient lies flat, and the surgical area and surrounding tissues are disinfected.

② Anesthesia: General anesthesia is generally used.

③ Make an incision at the site designed before surgery, peel off layer by layer and expose the bone tissue.

④ Osteotomy and movement and fixation of bone blocks: Cut the maxillary bone and move it forward to a suitable position to put the maxilla and mandible in an ideal occlusal state (for patients with combined maxillary and mandibular crossbite, the excess part of the mandible needs to be removed while moving the maxilla), and fix it. Titanium nails are generally used for fixation.

⑤ Suture: Rinse the wound repeatedly, and suture the periosteum, muscle and mucosal tissue in turn.

⑥ Apply pressure bandage.

6. Risks and complications

① Infection: Any surgery has the possibility of infection, especially the intraoral incision procedure. Eating and other factors will increase the possibility of infection. After the operation, you should take antibiotics for 5-7 days as prescribed by the doctor.

② Maxillary and mandibular fractures: When using an osteotome to strike the osteotome to remove harder bone tissue, excessive force may cause fractures.

③ Bleeding and postoperative hematoma: Bleeding is more during osteotomy and congestion is easily formed.

④ Nerve damage: Nerve damage may occur during the layer-by-layer peeling process.

⑤ Bilateral asymmetry: When removing bone tissue, it is possible to cause asymmetric removal of bones on both sides.

⑥ Unsatisfactory results: Too much bone tissue may be removed during the operation, resulting in other types of deformities, or too little bone tissue may be removed and fail to achieve the desired results.

⑦ At the same time, after the maxillary bone was moved forward, the mid-face depression was also significantly improved. For some patients who have a low nose after underbite correction, rhinoplasty can be performed to further increase the three-dimensional effect of the mid-face.
