Three major symptoms of venous thrombosis

1. Pain

Pain is the earliest symptom of deep vein thrombosis, mainly because the thrombus stimulates an inflammatory response in the vein wall and the vein distal to the thrombus expands rapidly, stimulating the peripheral nerve receptors in the blood vessel wall. The pain often occurs in the calf muscles, thigh, or groin area, but not in the feet or toes. The degree of pain varies depending on the extent of thrombosis, the severity of the inflammatory response, and the individual's sensitivity to pain.

Most patients complain of lower limb pain, painful cramps or tightness, which worsens after activity and can be relieved by bed rest or elevating the affected limb. Although symptoms develop relatively rapidly, patients rarely recall the exact time they occurred. Generally, after the pain appears, it gradually worsens and lasts for several days. Homans sign may be positive in some patients, that is, pain may be stimulated when the foot is dorsiflexed to tense the morpheus muscle.

2. Swelling

Swelling of the lower limbs is the main or only symptom. Except for a few cases with bilateral lower limb swelling due to inferior vena cava thrombosis, most of them have unilateral lower limb swelling. The degree of swelling depends on the degree and extent of the venous occlusion. If the swelling is located in the deep small veins, it is often difficult to detect; if it is located in the inferior iliac main vein, it can quickly cause venous blood return obstruction and obvious swelling.

Lower limb lesions often originate from the rectal venous plexus or the renal-femoral vein. Except for some thrombi that may dissolve or be confined to the site of onset, the remaining thrombi may extend proximally and distally to affect the entire deep vein trunk, manifesting as severe swelling of the lower limbs. Swelling below the knee suggests thrombosis involving the superficial femoral vein or femoral vein; swelling of the entire lower limb indicates thrombosis of the superficial femoral vein. Measuring the circumference of both legs can often help determine the extent of swelling. Normally, the circumference of both lower limbs should be less than 1 cm when compared in the same plane. After deep vein thrombosis, swelling may last for weeks or months, or even for a lifetime.

3. Superficial varicose veins

Superficial varicose veins are a secondary compensatory reaction to deep vein thrombosis. If the thrombosis involves the main deep veins, especially the iliac-femoral vein segment, it can cause obvious varicose veins in the lower abdomen and groin. Deep vein thrombosis can cause limb swelling, body pain, and varicose veins in patients, so attention should be paid to timely detection and timely treatment.
