What to do if your toes turn black due to vasculitis

Vasculitis is a relatively serious disease. After suffering from vasculitis, the patient's toes often turn black and cause ulcers, and severe cases even require amputation. The most important treatments for vasculitis include non-surgical treatment and surgical treatment. Conventional treatment requires quitting smoking and drinking, strengthening lifestyle conditioning, actively exercising, and paying attention to medication. If the condition is more serious, surgery can be used for treatment.

1. Non-surgical treatment ⑴ General treatment: In addition to no smoking, you should also prevent cold, moisture and trauma, and avoid overheating to avoid increased tissue hypoxia. Exercise the affected limb by performing lift-and-hang exercises, such as the buerper exercise, in which the patient lies flat on his back and first raises the affected limb more than 45 degrees, then lets it hang by the bed for 2-3 minutes, then places it in a horizontal position for 2 minutes, and performs foot rotation, flexion and extension exercises. Repeat this activity for 20 minutes, several times a day. Or simply walk, the walking speed and distance are both based on the standard of not causing lameness. ⑵Drug treatment a. Drugs that reduce blood viscosity, anti-platelet aggregation, hyperbaric oxygen therapy, etc. b. Vasodilators c. Antibiotics and sedatives d. Others: Traditional Chinese Medicine

2. The purpose of surgical treatment is only to promote or reconstruct limb arterial blood supply to improve or eliminate the consequences caused by ischemia. Surgery cannot be relied upon as a direct or specific treatment for this disease. (1) Arterial reconstruction and bypass surgery: As the name suggests, it is to use artificial or autologous blood vessels to replace the blocked arteries. This is currently the most classic method for treating thromboangiitis obliterans. There are two types of materials used for bypass surgery. One is artificial blood vessels, which are made of polytetrafluoroethylene (PTFE). Its advantages are good biocompatibility, larger tube diameter, and selectable size and length. The disadvantages are: it is expensive (usually 10,000 to 15,000 yuan), not suitable for bypassing small blood vessels, so it is generally used for vascular bypass surgery above the knee joint. The second is to use autologous great saphenous vein, which has the advantage of being taken from one's own body, without any rejection reaction, and having a small caliber, making it suitable for small blood vessel anastomosis. But the disadvantages are: the surgical wound is large and is limited by the patient's own conditions (some patients' great saphenous vein is too thin to be used for bypass).

(2) Interventional therapy Interventional therapy is equivalent to an intravascular surgery, which can achieve the effect of surgery without the need for surgery. Generally, for vascular occlusion above the knee joint, the first choice is balloon dilatation followed by stent implantation. This can reopen the blood vessels with minimal trauma and can be done using local anesthesia without the risks of anesthesia surgery. But the price is relatively expensive, generally the price of a bracket is between 12,000 and 15,000 yuan. (3) Stem cell transplantation is one of the latest medical technologies in the world. It is suitable for patients with chronic lower limb ischemic diseases caused by various reasons, such as vasculitis, arteriosclerosis obliterans, diabetic foot, etc., who have failed to respond to conservative treatments, and whose diseased blood vessels have no outflow tracts as shown by imaging examinations and cannot be surgically bypassed, or who have good outflow tract arteries and may be successfully bypassed, but are elderly and frail and cannot tolerate surgery. But its long-term effects remain to be tested by time.
