Disease Overview:

Herpetic vulvovaginitis is caused by herpes simplex virus type 2 and is mostly transmitted through sexual intercourse. It is less common than herpetic gingivitis. At the beginning, there is local pain and difficulty urinating. The mucous membranes of the vulva, vagina, and cervix are red and swollen, with white patches. Ulcers may also form, covered with gray-yellow pseudomembranes. There may be scattered blisters on the skin near the vulva. Inguinal lymph nodes are swollen and tender, and other symptoms may include fever, myalgia, or meningitis.

How genital herpes causes female infertility:

1. Vaginal infection caused by pathogens leading to infertility

Some vaginitis may be caused by infection with other pathogens. For example, ureaplasma urealyticum, Chlamydia trachomatis, Mycoplasma genitalium, etc. may cause vaginal infection. This infection will not only cause infertility, but it is also likely to cause fetal arrest, miscarriage, and stillbirth. Therefore, if you plan to get pregnant, you should conduct a careful examination to rule out the many factors that may lead to infertility.

2. Changes in the vaginal environment lead to infertility

The vagina is a receptor for sexual intercourse and semen. Under normal circumstances, the vagina has a relatively safe environment with a relatively constant acidity and alkalinity. When certain organic or functional diseases of the vulva and vagina occur, changes in the vaginal environment affect the function of normal sperm, leading to infertility. This environment, which is suitable for the temporary retention and passage of sperm, is very important. Once this environment is destroyed, infertility is likely to occur.

3. Vaginal bleeding leading to gynecological infertility

Vaginal bleeding is one of the most common symptoms of female reproductive diseases. The causes are multifaceted. In addition to normal menstruation, vaginal bleeding may occur when there is inflammation of the reproductive organs, pathological pregnancy, endocrine diseases, trauma, tumors, etc. Therefore, if a female friend experiences abnormal vaginal bleeding, she should go to the hospital for examination immediately to find out the cause of the bleeding to avoid causing serious gynecological diseases and leading to infertility.

Treatment:

1. Treatment with traditional Chinese medicine

For those with excessive heat, clear away heat and dampness, and use Longdan Xiegan Decoction with modifications; for those with excessive dampness, strengthen the spleen and eliminate dampness, and use Chushi Weiling Decoction with modifications; if local pain persists after the rash subsides, it is advisable to soothe the liver and regulate qi, promote blood circulation and relieve pain, and use Chaihu Shugan Yin or Jinlingzi San (Jinlingzi, Yanhusuo) with modifications. You can also use the proven recipe of 15g of isatis indigotica or isatis root, boiled in water and used as tea. 2 mL of Isatis root injection can be injected intramuscularly 1 to 2 times a day, with 10 times as a course of treatment. Patients with high fever, especially those with corneal herpes caused by the involvement of the trigeminal nerve, can take 0.1-0.5g of antelope horn powder orally.

2. Local treatment

The principles are to reduce inflammation, dry, astringe and prevent secondary infection. You can use 2% gentian violet solution or compound sanguisorba officinalis zinc oxide oil (raw sanguisorba officinalis 10.0, lithospermum officinale 5.0, borneol 2.0, zinc oxide oil added to 100.0) for external application. If there is secondary infection, neomycin ointment can be applied externally. In case of gangrenous ulcers, 0.1% neomycin solution or 0.1% ethacridine solution can be used for wet compresses. For herpes zoster ophthalmicus, 0.1% to 0.5% herpes net solution can be used for eye drops. Some people use 0.1% phosphoacetic acid cream for external use, which can relieve pain and shorten the course of the disease.
