Multinodular goiter Multinodular goiter may be caused by a mild synthesis disorder of thyroid hormone. Because of the decrease in serum T4, the pituitary gland secretes reactively, resulting in a slight increase in TSH, causing goiter. Because goiter is compensatory, thyroid function is normal in most patients. Most cases are clinically asymptomatic, with only enlarged thyroid gland and nodules found during physical examination. Patients often seek medical attention due to airway obstruction or difficulty swallowing. For those with mild symptoms, they should be carefully asked whether they have airway obstruction. The patient's thyroid gland may also be enlarged behind the sternum, with jugular venous distension and facial congestion when the arms are extended upward and a palpable retrosternal goiter is visible. A small number of older patients with multinodular goiter may also develop hyperthyroidism. 3. Congenital goiter It is a congenital disorder of thyroid hormone synthesis. In severe cases, it is sporadic cretinism, which is a myxedema syndrome in infancy, characterized by growth and development disorders, mental retardation, diffuse myxedema throughout the body, and obvious symptoms and signs of hypothyroidism. |
