Almost all patients have changes in vision and visual field. More than 80% of patients have the first symptom, and the visual impairment is mostly slow and progressive, which can last for months or years. In the early stage, unilateral vision loss is accompanied by temporal visual field defect, unilateral visual impairment accounts for 55%, and then the contralateral optic nerve and optic chiasm are compressed, which manifests as decreased binocular vision or bilateral visual field defect, bilateral visual impairment accounts for 45%, and finally it can lead to blindness. However, the changes in bilateral vision or visual field are often asymmetrical and irregular. In a very small number of patients, one side is blind while the other side is still normal. These patients often visit the ophthalmology department first. In addition, due to compression of the optic nerve and optic chiasm, primary optic disc atrophy often occurs in the fundus, which can be as high as 80%. In the late stage, secondary fundus edema may also occur simultaneously due to increased intracranial pressure. 2. Symptoms of involvement of adjacent structures: When the olfactory tract is affected, the sense of smell is reduced or disappeared on one or both sides. When the frontal lobe is involved, mental symptoms such as drowsiness, memory loss, and anxiety may occur. When the cavernous sinus is compressed, it may cause oculomotor nerve paralysis and exophthalmos. 3. Pituitary and hypothalamic dysfunction: Symptoms of pituitary endocrine dysfunction and hypothalamic damage are less common, but when the tumor grows and compresses the pituitary gland, symptoms of decreased pituitary function may also occur, such as decreased libido, impotence or amenorrhea; when the hypothalamus is affected, polyuria, polyuria, obesity and drowsiness may also occur. |
