Is Joben's hyperthyroidism serious?

Hashimoto's hyperthyroidism is a type of hyperthyroidism. Since it was discovered by Japanese Dr. Hashimoto, this type of hyperthyroidism was named Hashimoto's hyperthyroidism. There are many things to pay attention to during the treatment process of Hashimoto's hyperthyroidism. Remember to eat more fruits and vegetables. You should be careful not to overwork yourself. Overwork can increase the symptoms of hyperthyroidism and even cause dizziness and nausea. So, is Hashimoto's hyperthyroidism serious?

1. Endocrine system also has a certain influence on Hashimoto's hyperthyroidism. Prolactin (PRL) receptors are present on the immune cells of central and peripheral immune organs and peripheral blood vessels. PRL is involved in the regulation of the body's cellular and humoral immune responses. PRL can not only act as a cell mitogen to assist interleukin-2 (IL-2) in stimulating the proliferation and differentiation of lymphocytes, but also stimulate the activity of ornithine decarboxylase and protein kinase C to promote lymphocyte activation. Once the endocrine system is out of balance, it will lead to abnormalities in humoral and cellular immunity and hinder the activation of lymphocytes, which is also one of the factors and mechanisms causing Hashimoto's hyperthyroidism.

2. Inspection

Typical symptoms include hyperthyroidism, such as shaking hands, heat intolerance and sweating, irritability, palpitations, insomnia, weight loss, and increased bowel movements.

Laboratory tests: decreased thyroid stimulating hormone (TSH), increased serum free tetraiodothyronine (FT4) or serum free triiodothyronine (FT3), or both increased;

A single serum thyroglobulin antibody (TGAb) increased by more than 3 times, or a thyroid peroxidase antibody (TPOAb) increased by more than 3 times, or both increased by more than 3 times;

Ultrasound manifestations: The thyroid gland is enlarged to varying degrees, and small dark areas can be seen in the uneven interior. The typical manifestations are obvious enlargement, thickening of the isthmus, irregular shape, concave-convex capsule, significantly reduced echogenicity, and grid-like distribution of enhancing cords. Some patients may have nodules with various echogenicities. In addition, for patients with atypical clinical manifestations and low or negative antibody titers, fine needle aspiration cytology or tissue biopsy can be performed to confirm the diagnosis.

3. Treatment principles

Western medicine treatment

1. Patients with mild symptoms: If FT3 and FT4 do not increase significantly, TSH does not decrease significantly, and the symptoms last for a short time, antithyroid drug treatment may not be given.

2. Patients with more severe symptoms: If FT3 and FT4 are significantly elevated and TSH is significantly decreased and the symptoms last for more than 1 week, antithyroid drugs should be given. The types of antithyroid drugs used and the method of administration should be smaller than the normal conventional doses, and attention should be paid to regular review of changes in FT3, FT4, and especially TSH. Adjust the drug dosage and method of administration at any time, reduce the dosage or stop the medication when appropriate, and be alert to secondary hypothyroidism.

3. Patients with suspected cancer or obvious compression symptoms: surgical treatment is given, and frozen pathology is performed during the operation. The surgical method is determined based on the pathological results. The options include bilateral subtotal thyroidectomy, local resection of thyroid nodules, and subtotal thyroidectomy. If the tumor is malignant, conventional radical surgery for thyroid cancer will be performed.
