The lesions can affect both the central and peripheral nerves, with the latter being particularly common. Distal sensory neuropathy is the most common lesion, accounting for more than 50% of all diabetic neuropathy. The manifestation of sensory symptoms is related to the size of the affected nerve fibers. If it is a small fiber, pain and paresthesia are the main symptoms. The pain can be dull, burning, stabbing, or cutting, and most of the pain is aggravated at night. 4. Intercostal neuralgia Intercostal neuralgia refers to one or several intercostal areas radiating from the back along the intercostals to the anterior wall of the chest and abdomen, distributed in a semi-circular shape.

3. It is usually unilateral, but bilaterally affected. The pain is often made worse by coughing, taking deep breaths, or sneezing. Physical examination may reveal tenderness and percussion pain in the thoracic spinous processes, between the spinous processes or beside the vertebrae. A few patients may have tenderness along the intercostals and paresthesias in the areas innervated by the affected nerves. The pain is usually stabbing or burning, and radiates along the intercostal nerves.
