The type of molar impaction also has a certain impact on the occurrence of pericoronitis. Clinically, partial vertical impaction and mesial oblique impaction are the most common and most often cause pericoronitis. The position of incompletely erupted tooth crowns is mostly lower than the occlusal plane of the second molar. The distal, buccal and lingual sides are often covered by gingival flaps, forming a deep and narrow blind pocket between the gingival flap and the crown. It has poor self-cleaning effect and is easy to hide food residues. Its temperature and humidity are good conditions for bacterial growth and reproduction. Under normal circumstances, the human body has strong resistance and bacteria cannot succeed. However, when the human body has a cold, lacks sleep, is fatigued, etc., the body's resistance is reduced, and bacteria multiply and cause infection. Mechanical damage to the gingival flap during biting and chewing causes ulceration of the mucosa, destroys the defense function of the tissue, allows bacteria to enter, and can also cause pericoronitis. The clinical manifestations of pericoronitis are initially painful and swollen gums, which worsen during chewing and swallowing. The pain may worsen when the mouth is opened. When the infection spreads to the masseter muscles and medial pterygoid muscles, the jaw may be locked. Local swelling may occur, and the lymph nodes may become enlarged and tender. Systemic symptoms may appear at this time, such as general discomfort, fever and increased white blood cell count. If the pain persists and the fever persists within 2 to 3 days of onset, it may be considered that the inflammation has developed into the suppurative stage. If incision and drainage are performed in time during this period, the inflammation will gradually subside. If the infection is not controlled during this period, the inflammation will spread. The infection can spread to the masseter muscles, cheeks, parapharyngeal muscles, mandible, etc., causing interstitial infection in the corresponding spaces.
