The clinical manifestations of acute eczema are rapid onset, short course, and initial local skin symptoms of erythema, flushing, heat, and swelling, followed by scattered or dense papules or pox on the erythema, exudation of fluid, and constant itching. Severe cases are accompanied by fever, thirst, irritability, constipation, short and dark urine, red tongue, thin white or yellow fur, and a stringy and slippery or stringy and rapid pulse. The syndrome is characterized by internal accumulation of dampness and heat, with the heat being more severe than the dampness, which is trapped in the blood and distributed to the skin. It should be treated by clearing away heat and dampness while cooling the blood.

The prescription is Longdan Xiegan Decoction with modifications: 10 grams each of gentian, yellow taro, and moutan bark, 20 grams of Imperata root, 15 grams of Rehmannia glutinosa, 10 grams of honeysuckle, 20 grams of plantain, 30 grams of gypsum, and 30 grams of Liu-san. Decoction in water. Add or subtract: If systemic symptoms such as fever, thirst, and irritability are obvious, add 0.2-0.3 grams of antelope horn powder (or 3 grams of raw hawksbill turtle). For severe itching, add 10 grams each of white moss peel and Kochia scoparia seeds; for constipation, add 6 to 10 grams of Sichuan peppermint.

If the onset is slower, the rash is mainly papules or blisters, the skin is flushed, the degree is lighter than the former, the itching is severe, and there is erosion after scratching, with more exudation, accompanied by fatigue, poor appetite, loose or dry stool, clear and long urine, pale tongue, white or greasy tongue coating, slippery or stringy or slow pulse, it is a sign of internal accumulation of dampness and heat, with dampness being heavier than heat. The treatment should be to strengthen the spleen and eliminate dampness, assist with clearing heat. The prescription is based on experience: 12 grams each of red Poria cocos and Atractylodes macrocephala, 10 grams each of Rhizoma Coptidis, Rhizoma Cibotii, Gentiana scabra, Rhizoma Cibotii, Rehmannia glutinosa, and bamboo leaves, 3 grams of rush, 15 grams of Liuyi powder, and 20 grams of Plantago.
