Calculous cholecystitis is a disease in which stones form in the gallbladder or in the neck of the gallbladder. The clinical manifestations depend on the location and size of the stone, whether it causes infection or obstruction, and the location and degree of obstruction. Calculous cholecystitis is the most common lesion in the biliary system. Depending on the location, there can be gallbladder stones, primary or secondary common bile duct stones, extrahepatic bile duct stones or intrahepatic bile duct stones.

Symptoms of calculous cholecystitis generally include the following:

1. Gallstones in the gallbladder

Generally, it does not produce symptoms of biliary colic, which is called dormant stones. It may have symptoms of indigestion such as dull pain in the right upper abdomen, acid reflux, belching, and abdominal distension, which are more obvious after taking greasy food. If accompanied by infection, it may have symptoms of acute cholecystitis. There may be no positive physical signs, and in a few cases where the stones are too large, the gallbladder may be palpable.

2. Gallstones in the cystic duct

There may be symptoms of biliary colic, sudden persistent pain in the right upper abdomen, paroxysmal aggravation, radiating to the shoulders and back, accompanied by nausea, vomiting, etc. For detailed consultation points, please refer to acute and chronic cholecystitis. There is tenderness in the right upper abdomen and positive Murphy's sign. An enlarged gallbladder with tenderness can be felt in the right upper abdomen. If gallbladder perforation occurs, there may be signs of acute peritonitis such as tenderness in the whole abdomen, rebound tenderness, and muscle tension. Some patients may develop jaundice.

The clinical manifestations of calculous cholecystitis are:

1. White blood cells

Total number>10×109/L, nuclear shift to the left.

2. Abdominal X-ray

Positive stones can be seen in the gallbladder area on X-ray.

3. Ultrasound examination

The gallbladder was enlarged, with a wall thickness >3.5mm and a bright mass with acoustic shadow inside.

4. Intravenous cholangiography

The gallbladder was not visualized.

5. CT or MRI

Showing gallstones.
