Bile reflux gastritis symptoms

Clinical symptoms

1. Abdominal distension: This is one of the main symptoms, manifested as abdominal fullness and discomfort, persistent burning sensation in the upper and middle abdomen, and may also manifest as pain behind the sternum, which may worsen after meals.

2. Heartburn: There is a burning sensation in the stomach, and some patients even have a burning sensation in the esophagus, often accompanied by belching, acid reflux, nausea, vomiting, intestinal rumbling, difficulty in defecation, loss of appetite, and weight loss.

3. Gastric bleeding: Patients with severe bile reflux gastritis may also experience gastric bleeding. Some patients have black stools, and some vomit blood.

4. Vomiting: Due to gastric emptying disorder, vomiting usually occurs in the evening or midnight, and some patients have blood in their vomit.

Diagnosis

Gastroscopy: It can be directly seen that there is a lot of gastric juice, which is grass green, and the gastric mucosa is congested, edematous, or eroded; the pyloric opening is open, and bile refluxes from the duodenum through the pylorus to the stomach. Pathological biopsy indicates gastritis.

Gastric aspirate measurement: insert a gastric tube to aspirate fasting and postprandial gastric juice, and measure the bile acid content. If the fasting basal gastric acid secretion (BAO) is <3.5mmol/h and the bile acid exceeds 30μg/ml, bile reflux gastritis can be diagnosed.

Isotope determination: 2mCi99mTc-butylimide diacetate was injected intravenously, and the liver and bile duct were observed every 5 minutes for a total of 1 hour. Every hour the patient drinks 100 ml of water containing 0.3 mCi99mTc to accurately determine the position of the stomach. Then, for 2 hours, check the liver, gallbladder, and stomach area every 15 minutes to determine the gastrointestinal reflux index. The normal value is 8.6±6.0; in patients with reflux gastritis, it increases to 86.3±7.1. A 99mTc-labeled solution can also be injected into the duodenum or upper jejunum, and the isotope content in the stomach can then be recorded to understand the extent of gastrointestinal reflux.

Patients with bile reflux gastritis have a feeling of fullness or discomfort in the stomach, which is often aggravated after meals, or have stomach pain, or a cold stomach, which may be accompanied by abdominal distension, belching, acid reflux, heartburn, nausea, vomiting, gastric gurgling sounds, intestinal rumbling, difficulty in defecation, loss of appetite and weight loss. In severe cases, there may also be gastric bleeding, manifested as vomiting blood or black stools (tarry stools) and a positive fecal occult blood test.

The occurrence of bile reflux gastritis requires the basic conditions of loss of pyloric function or incomplete pyloric closure. For example, after gastrectomy or gastrointestinal anastomosis, bile can directly reflux into the stomach; some patients have no history of surgery, and the duodenal contents can reflux into the stomach through the incomplete closure of the pylorus, causing reflux gastritis; after cholecystectomy, the function of storing bile is lost, and bile continues to flow into the duodenum. If it refluxes into the stomach through the incomplete closure of the pylorus, it can also cause reflux gastritis.

It is currently believed that bile reflux can lead to upper gastrointestinal ulcers, gastrointestinal inflammation and tumors. In other words, long-term bile reflux can lead to hyperplasia, active inflammation, esophagitis, gastric mucosal erosion, gastric ulcers, and even promote the occurrence of gastric cancer. It is closely related to the occurrence of ulcers, upper gastrointestinal inflammation, and tumors. If bile reflux gastritis is not treated in time, serious complications may occur, such as ulcers, gastrointestinal bleeding, esophageal stenosis, etc. The refluxed gastric juice can also erode the vocal cords, pharynx and trachea, causing chronic laryngitis, chronic pharyngitis and tracheitis.
