Diagnosis and treatment of common anorectal diseases

(1) Symptoms:

① Intermittent blood in stool: characterized by dripping and spurting of blood during bowel movements, with large amounts of blood and bright red color, and the blood cannot be confused with feces. It may also manifest as blood on toilet paper.

② Prolapse: After defecation, granular swellings protrude from the anus. In the early stage, they can be retracted by themselves, but in the later stage, they need to be supported by hands or rest in bed to be restored. In severe cases, they may protrude when squatting, walking, coughing or sneezing.

③ Anal discomfort: including anal distension, foreign body sensation, itching or pain, which may be accompanied by mucus discharge.

④ Anal pain: People with anal edema or thrombosis may experience anal pain.

(2) Physical signs:

Rectal examination revealed that the mucosal skin was raised in the same position above and below the dentate line, which was connected into a whole, soft in texture, and mostly located at 3, 7, and 11 o'clock.

The stool of rectal cancer contains blood and mucus, is dark red in color, and the frequency and nature of bowel movements change. Rectal examination may reveal a hard mass with an uneven surface. Endoscopic examination showed that the tumor was cauliflower-like, with eroded surface, brittle texture, and easy to bleed when touched. Pathological examination showed mostly adenocarcinoma.

Operation method: After satisfactory anesthesia, appropriately dilate the anus, and perform a comprehensive anal examination with digital examination and anoscopy. Select the surgical incision reasonably according to the size and distribution of the hemorrhoids. Lift the base of the external hemorrhoids with curved forceps, and sharply separate the external hemorrhoidal tissue radially to 0.5 cm above the tooth line. Make a fusiform incision, lift and clamp the internal hemorrhoids at the corresponding position with large curved forceps, and ligate the internal hemorrhoidal tissue with No. 7 thread. Patients with multiple surgical incisions should pay attention to preserving the anal skin bridge between the incisions, trimming the surgical incisions, ligating to stop bleeding, inserting gauze strips to secure the anus, and applying pressure bandages to fix.
