Characteristics and prevention points of tremor paralysis

Perioperative Risks

1. The commonly used therapeutic drug levodopa has a short half-life, and stopping treatment before surgery can cause Parkinson's crisis. Drugs such as monoamine oxidase inhibitors, dopamine antagonists, and catecholamine methyltransferase inhibitors can cause risks such as perioperative impaired consciousness, orthostatic hypotension, hallucinations, delirium, and ataxia.

2. Such patients have upper respiratory tract muscle dysfunction, which can easily lead to complications such as aspiration, secretion discharge disorder and atelectasis; about 1/3 of patients have obstructive ventilation disorder.

3. Laryngeal spasm and respiratory insufficiency may occur after extubation under general anesthesia.

4. Patients who use levodopa for a long time may experience significant hypotension or hypertension during anesthesia induction. The reasons include relative blood volume deficiency, catecholamine depletion, autonomic dysfunction and sensitivity to catecholamines; the heart is irritable and arrhythmias may occur.

Perioperative strategies

1. Parkinson's disease patients' drug treatment should continue until the morning of the day of surgery, and levodopa should be re-administered as soon as possible after surgery. The perioperative use of other types of drugs should be consulted with a neurologist.

2. Avoid the use of drugs with anti-dopaminergic effects such as phenothiazines, butyrophenones and metoclopramide.

Key points of anesthesia

1. Levodopa can be taken orally 20 minutes before induction, and can also be repeatedly administered through a gastric tube during the operation.

2. Avoid the use of droperidol, alfentanil, and ketamine.

Prevention of this disease:

1. Persistent exercise (especially Tai Chi and Ba Duan Jin) is beneficial to the smooth flow of Qi and blood, the smoothness of tendons and veins, and the regulation of brain nerves.

2. Eating a light diet and preventing and treating high blood pressure, diabetes, and stroke can all be beneficial in fighting brain degeneration.

3. Regulate emotions, avoid anxiety and depression, and maintain a peaceful and happy state of mind.
