What to do if you have long-term hematuria

Long-term brainwashing will pose a threat to the body and health, so it is recommended that patients first determine whether it is true or false hematuria, and also pay attention to the nature of the lesion and the different sites of bleeding, so as to make judgments and understand and use symptomatic medications.

1. Determine true or false hematuria

Sometimes the blood in the urine seen with the naked eye is not necessarily real blood in the urine, so the first step is to determine whether it is real or not. First of all, hematuria should be distinguished from hematuria caused by menstrual, uterine, vaginal bleeding or hemorrhoidal bleeding, which contaminates urine, as well as other factors.

Second, hematuria should be distinguished from hemoglobinuria: the main methods are naked eye observation, microscopic examination, and positive benzidine test

Third, the difference between hematuria and porphyria:

In case of porphyria caused by pyrrole metabolism disorder or lead poisoning, a large amount of porphyrin may be produced, causing porphyrinuria. At this time, relevant examinations should be carried out. The benzidine test is negative and the urine violet bile test is positive.

Fourth, hematuria should be distinguished from red urine caused by certain drugs and dye reagents, such as amitraz, santolin, rhubarb, Congo red, phenol red and other drug pigments. The absence of red blood cells in the urine and a negative benzidine test indicate the drug's effect.

2. Determine the bleeding site and the nature of the lesion

(I) Analysis based on the relationship between hematuria and urination

If it is initial hematuria, it may be caused by urethral lesions, such as urethral injury, tumor, caruncle, prostatitis, etc. If it is terminal hematuria, it may be due to lesions in the bladder neck, trigone, posterior urethra, cystitis, bladder tumor, stones, prostate lesions, etc.

If there is hematuria throughout the whole process, it may be an upper urinary tract disease.

(II) Different bleeding sites and different conditions

1. Characteristics of hematuria caused by kidney disease

(1) Hematuria is continuous, uniform, and often dark brown in color;

(2) Often accompanied by dull pain or colic in the kidney area;

(3) The blood clot is in the form of strips, and sometimes red blood cell casts or other casts can be found;

(4) No urinary tract irritation and difficulty urinating.
