II. Systemic treatment 1. Oral or injection of antihistamines, 5-hydroxytryptamine antagonists and sedatives. Such as chlorpheniramine (chlorpheniramine), diphenhydramine, cyproheptadine, cetirizine, astemizole, terfenadine, loratadine, cimetidine (cimetidine), ranitidine, *** (diazepam), etc. These drugs can be selected according to the condition. H2 receptor antagonists alone have a certain effect on the pruritus caused by Hodgkin's disease and polycythemia. 2. For those with severe itching, 10% calcium gluconate can be injected intravenously or 0.25% procaine hydrochloride and vitamin C 1.0-2.0g can be added to 500ml of normal saline or compound sodium chloride solution and dripped intravenously. Naloxone can be used for patients with intractable pruritus, but it is potentially addictive. 3. For menopausal or senile pruritus, male patients can choose to use 50 mg of testosterone propionate intramuscular injection 2 to 3 times a week, or 25-50 mg of nandrolone phenylpropionate tablets intramuscular injection 2 to 3 times a week; female patients can take 1 mg of diethylstilbestrol orally once a day. 4. For those with dry skin, vitamin A 50,000 U/d can be taken orally or injected. 5. For itching caused by cholestatic diseases or uremia, cholestyramine (cholestyramine) can be used, 5 to 8 g per day. 6. Rifampicin can be used for itching caused by primary biliary cirrhosis, but it is toxic to the liver, so further observation is needed to determine its safety. 7. For itching with obvious mental factors, tricyclic antidepressants such as doxepin can be used. 3. Local treatment 1. For those with mild secondary damage, 1% calamine carbonate lotion, antipruritic lotion, various steroid emulsions (such as 0.025% fluocinolone emulsion, 0.25% fludrocortisone emulsion, 5% benzocaine emulsion or lotion, 1%-3% borneol emulsion, 5%-10% black pomegranate oil emulsion or ointment, etc.) can be used externally. 2. For local secondary skin lichenification, infiltration and hypertrophy, refer to the local treatment of chronic eczema and neurodermatitis. |
