Sometimes people will find a lump growing in the gums. This lump is caused by apical periodontitis. When suffering from apical periodontitis, the patient will feel tooth pain and a feeling that the teeth are floating.

1. Infection: Like pulpitis, infection is also the most common cause of apical periodontitis, and most infections come from pulpitis. If pulp inflammation is not treated in time, the infection will spread deep into the root and cause apical periodontitis through the apical foramen. In addition, there are a few infections from periodontal disease. For example, when there is deep periodontal inflammation, it can cause apical periodontitis. Sometimes the infection comes from the infected root canal. For example, when doing root canal treatment due to pulpitis, the instrument exceeds the apical foramen when the root canal is enlarged with a needle and file, and the pressure is too high when flushing the root canal, and the liquid medicine, especially hydrogen peroxide, brings the infected substances in the root canal into the periapical tissue.

2. Chemical stimulation : When performing root canal treatment for pulpitis, pulp tissue devitalizing agents such as arsenic trioxide are used. The sealing time is too long, and highly irritating disinfectants and formaldehyde-cresol are sealed in the root canal. The drug overdose penetrates into the periapical tissue and can cause drug-induced apical periodontitis. 3. Trauma: Excessive biting force or external force on teeth may damage the periapical tissues. During root canal treatment, root canal instruments often extend beyond the apical foramen, and very hard root canal fillings extend beyond the apical foramen, both of which are traumatic factors to the periapical tissues.

4. Non-pulpal periapical lesions: Most periapical lesions are caused by the continued development of pulp disease, but some are not caused by pulp pressure lesions. In addition to the trauma and periodontal disease mentioned above, there are a few cases caused by other reasons, such as radiation osteonecrosis, central giant cell granuloma, malignant lymphoma, developmental cysts, etc.
