1. Hematoma of perineum and vaginal wall

Overview: Perineal and vaginal wall hematomas occasionally occur after delivery and in the puerperium, especially after assisted delivery, and can occur within a few hours, days, or 1 to 3 weeks after delivery.

Medical history: history of vaginal delivery, especially operative vaginal delivery, such as forceps, vacuum extractor or breech delivery, or complications of gestational hypertension or perineal laceration. When the perineal hematoma grows rapidly, the mother will feel severe swelling and pain in the perineum. When vaginal wall hematoma compresses the rectum, there may be a feeling of anal swelling and unbearable pain.

Physical examination shows that those with heavy bleeding have anemia and an acute and painful expression. Perineal hematoma can be seen as a local elastic and tough mass, the surface skin and mucous membrane are purple or purple-blue, and there is obvious tenderness. Vaginal wall hematoma can be seen as swelling of the vaginal wall on one side, with the mass partially or completely filling the vaginal cavity and protruding into the rectum or vulva.

2. Postpartum constipation

Overview: Postpartum bowel movements are difficult or last for several days because the stool stays in the colon for too long, absorbing too much water, making the stool dry and hard, and difficult to excrete. It may also be caused by fear of defecation due to pain from hemorrhoids or perineal wounds, leading to constipation.

Medical history: During the postpartum or puerperium period, the patient had a normal diet and no abdominal pain or vomiting. Difficulty in defecation, which may last for several days, or dry and painful stool that is difficult to defecate, and unbearable swelling in the anus.

Physical examination showed good mental and nutritional status, and no obvious distension or intestinal shape in the abdomen. Anal examination may reveal external hemorrhoids or mixed hemorrhoids, and dry and hard stools may be felt in the rectum.
