Why is it difficult to ejaculate?

Difficulty in ejaculation means that a man's penis can erect normally and he can have normal sexual intercourse, but it is difficult for him to ejaculate. This will cause the man to be unable to reach orgasm. Not ejaculating and not having difficulty in ejaculating are also important causes of infertility. Long-term non-ejaculation will cause men to have a loss of libido, the onset of some diseases, and it will also easily affect the relationship between couples.

Why is it difficult to ejaculate?

The common clinical causes of anejaculation are as follows:

1. Functional non-ejaculation

Some people exaggerate the harmfulness of normal masturbation, forming a psychological burden; or they masturbate excessively. Since masturbation stimulates the genitals more strongly than sexual intercourse, and masturbation may be accompanied by sexual fantasies, some people become dependent on masturbation, which increases their threshold of sexual excitement, so that contact with the vagina does not provide sufficient stimulation; or some people are in what psychology calls the "childish narcissistic stage"; some people believe that semen is the most essential substance in the human body, and ejaculation is harmful to the body. Over time, even if they want to ejaculate, they cannot; some people do not want children for fear that the woman will become pregnant; or the couple is in discord, and they are dissatisfied with their wife and do not ejaculate; some people do not want children, but are unwilling to take contraceptive measures and use the method of external ejaculation. When they want to have a child, they cannot ejaculate in the vagina.

2. Lack of sexual knowledge

Many men do not know that they need to thrust quickly after vaginal insertion in order to ejaculate. Instead, they do not thrust after vaginal insertion, or they move gently and slowly a few times, so that the local stimulation does not reach the intensity required for ejaculation. Either the woman restricts the man's thrusting because of pain during sexual intercourse, or the man has phimosis, a tight foreskin or a short frenulum, and it hurts when thrusting so he dares not thrust hard. Or the man may adopt a bad sexual posture, which is not conducive to rapid thrusting. For example, the man's upper limbs are in a vertical position, an arc-shaped bent position, or a straight forward position, instead of using his elbows to support the body; or his legs are placed on both sides of the woman's body, which are not conducive to large-scale rapid thrusting, and thus the intensity of ejaculation cannot be achieved. Or the mind is distracted and cannot concentrate on sexual activities.

3. Drug-induced anejaculation

For example, some antihypertensive drugs, antipsychotic drugs or sedatives can also cause anejaculation.

4. Excessive sexual activity

Some people have sex too frequently (such as having sex multiple times a night, etc.), and because of the refractory period, they fail to ejaculate. At this time, you only need to stop having sex for a while to resume ejaculation.

5. Organic anejaculation

Anejaculation caused by organic reasons is relatively rare in clinical practice. For example, spinal cord injury: in complete upper motor neuron injury, 70% of patients can have an erection, but few can ejaculate; in complete lower motor neuron injury, both erection and ejaculation are affected in most patients. Other spinal cord diseases such as spina bifida, spinal cord tumors, syringomyelia, or after retroperitoneal lymph node dissection can cause anejaculation. Abnormalities of the higher centers (brain), such as lesions in the lateral lobe of the brain, result in neither ejaculation nor orgasm. At this time, the physical examination, endocrine and X-ray examinations are all normal, but the bladder pressure curve is hypotonic. In most patients, the bladder pressure fluctuates significantly when deep breathing starts or stops. This respiratory effect indicates that there is an abnormality in the higher centers that control the bladder. The absence of ejaculation is also caused by abnormalities in the higher centers.
