There is a lump not far from the anus

In life, I believe many people have encountered a lump close to the anus. This is mainly due to hemorrhoids. If you want to treat hemorrhoids, you can use hemorrhoid cream, but you must pay attention to the hygiene of the anus to avoid bacterial infection. For the treatment of a lump not far from the anus, you can learn about the symptoms and treatment methods introduced in the article and correctly deal with the hemorrhoid condition.

Hemorrhoids Symptoms

Clinical manifestations

1. The most common sites for internal hemorrhoids are 3, 7, and 11 o'clock in the lithotomy position. The main manifestations are bleeding and prolapse. The common clinical symptom of internal hemorrhoids is intermittent bleeding after defecation. Some patients may also have difficulty defecating. Pain occurs when internal hemorrhoids are combined with thrombosis, incarceration, and infection. Internal hemorrhoid grading standards: [2-3] Grade I: blood or dripping blood during defecation, bleeding stops on its own after defecation, and the hemorrhoids do not protrude from the anus; Grade II: frequent blood in the stool, hemorrhoids protrude during defecation, and can be reduced in size on their own after defecation; Grade III: occasional blood in the stool, hemorrhoids protrude during defecation or when standing for a long time or bearing heavy loads, requiring manual assistance to reduce; Grade IV: occasional blood in the stool, hemorrhoids cannot be reduced after protrusion or they protrude again after reduction. May be accompanied by erosion of the dentate line mucosa, exposed small blood vessels, anal fissures, etc.

2. External hemorrhoids occur outside the anus, causing pain when going to the toilet, sometimes accompanied by itching. Common external hemorrhoids are mainly connective tissue hemorrhoids (skin tags, skin tags) and inflammatory external hemorrhoids.

3. Mixed hemorrhoids are the most common clinical form of disease. The symptoms of internal hemorrhoids and external hemorrhoids may exist at the same time, mainly manifested as blood in the stool, anal pain and swelling, anal itching, etc.

Treatment principles

There are three principles for the clinical treatment of hemorrhoids:

(1) Asymptomatic hemorrhoids do not require treatment;

(2) Symptomatic hemorrhoids are being treated to reduce or eliminate symptoms rather than cure them;

(3) Non-surgical treatment is the main approach. Current treatments for hemorrhoids can be divided into conservative treatment, outpatient surgical treatment, and surgical treatment. Most patients found to have low-grade internal hemorrhoidal disease will respond to conservative medical management. Conservative treatment options include dietary intervention (such as increasing fiber intake and drinking more water), lifestyle changes (such as developing bowel habits) and drug treatment (oral and external use of Chinese medicine, sitz baths with medicated liquids), etc.

If conservative treatment fails, traditional outpatient surgery can be chosen, such as traditional sclerotherapy, transparent cap-assisted endoscopic sclerotherapy, rubber band ligation, ultrasound Doppler-guided hemorrhoidal artery ligation, laser photocoagulation, bipolar electrocoagulation, cryotherapy, infrared coagulation therapy, etc.

. A recently emerging painless and minimally invasive treatment technology is transparent cap-assisted endoscopic sclerotherapy, which is quite popular among doctors and patients.

Sclerotherapy is suitable for patients with grade I and II bleeding internal hemorrhoids. Commonly used sclerosing agents include lauromacrogol injection, Chinese patent medicine injection, etc. After the injection of the sclerosant, a sterile inflammatory reaction will occur around the hemorrhoids and hemorrhoidal masses. After the submucosal tissue becomes fibrotic, the hemorrhoidal masses will shrink, thus achieving the treatment goal. Traditional sclerotherapy can be completed in an outpatient setting and is characterized by simplicity and convenience, but iatrogenic injury caused by ectopic injection is its biggest limitation.

Special reminder:

Any treatment for hemorrhoids needs to include treatment for other causes of hemorrhoids, such as diarrhea, constipation, and poor bowel habits.

Many people underestimate anal bleeding and mistakenly think it is hemorrhoids, which results in them being diagnosed with colon cancer. It is important to have a doctor evaluate and make a professional diagnosis.

The treatment of hemorrhoids should not be excessive, especially for ulcerative colitis, Crohn's disease, and Behcet's disease combined with hemorrhoids. Traumatic treatment methods should be avoided as much as possible.

It is recommended to complete a colonoscopy before treating hemorrhoids to effectively rule out the possibility of other diseases.

The efficacy evaluation after hemorrhoid treatment is mainly based on symptom evaluation, and evaluation through anoscopy and colonoscopy should be minimized.
