What causes cholecystitis?

Most cases of cholecystitis are caused by the presence of stones in the gallbladder, which block the cystic duct and prevent bile from being discharged smoothly, leading to bacterial infection and cholecystitis. In some patients, there are no stones in the gallbladder, but cholecystitis is caused by bacteria entering the gallbladder from the intestines or blood circulation. Patients with cholecystitis are also prone to developing gallstones due to changes in bile composition and bile concentration, which is mainly composed of bacteria and inflammatory necrotic substances. Therefore, cholecystitis and gallstones often coexist.

Why does the gallbladder become easily inflamed?

(1) The gallbladder is the enlarged part at the end of the cystic duct, which can hold 30 to 60 ml of bile. Bile must pass through the cystic duct to enter or be discharged from the gallbladder. The cystic duct is about 3 to 4 cm long and 2 to 3 mm in diameter. The mucosa inside the cystic duct forms 5 to 7 spiral folds, making the lumen relatively narrow, which makes it easy for gallstones and parasites to embed in the cystic duct. Once embedded, the bile in the gallbladder cannot be discharged. As a result, excess bile accumulates in the gallbladder, stays there for a long time and becomes overly concentrated, directly irritating the gallbladder mucosa and causing inflammation.

(2) The blood vessels that supply nutrition to the gallbladder are terminal arteries. When the gallbladder's outlet is blocked, the gallbladder mucosa continues to secrete mucus, causing the pressure inside the gallbladder to continue to increase, causing the gallbladder to swell and accumulate water. As a result, the blood vessels in the gallbladder wall are compressed, causing ischemia and necrosis. When the gallbladder is ischemic, the resistance of the gallbladder decreases, and bacteria can easily grow and multiply, taking the opportunity to become active and causing cholecystitis.

(3) Since the gallbladder has the function of storing and concentrating bile, the contact time between the gallbladder and bile is longer than that of other bile ducts, and the concentration of bile in contact is also high. When bacteria are present in the human bile duct at this time, infection will occur, and the chance of developing cholecystitis will of course increase.
