A small number of patients develop neurotrophic disorders due to the long course of the disease, resulting in thenar muscle atrophy, intermittent skin paleness and cyanosis, and in severe cases, cyanosis of the thumb and index finger, necrosis of the fingertips, or atrophic ulcers. During the examination, you can tap the middle of the palm side of the wrist, which will cause numbness and pain in the area innervated by the median nerve. This is a positive Tinel sign. Some patients experience increased paresthesia in their fingers after extreme flexion of the wrist for 60 seconds, which is a positive Phalen test. Symptoms can be induced by applying pressure to the upper arm using a sphygmomanometer until the distal limb veins dilate.
