(1) Herpes simplex keratitis (HSK) [Overview] Herpes simplex keratitis is a serious blinding eye disease, the pathogen of which is the herpes simplex virus. The virus is divided into two serotypes: type I and type II. Most corneal lesions are caused by type I, and a few are caused by type II. The patient is prone to this disease when his body resistance is reduced, such as when he has a cold, fever, fatigue or severe mental stimulation. 【Clinical manifestations】

1. Primary infection is more common in young children. There is fever, swollen and painful lower anterior lymph nodes, and herpes on the lips and wings of the nose. When the eyes are affected, they manifest as acute follicular conjunctivitis and eyelid skin herpes. At this time, about 2/3 of patients have punctate or tree-shaped keratitis, and less than 10% suffer from stromal keratitis and uveitis.

2. Recurrent infection is more likely to occur when the body's resistance is reduced. Corneal lesions may be punctate, tree-shaped, or map-shaped ulcers; in severe cases, stromal keratitis, corneal melting, and perforation may occur; corneal sensation in the affected area may be reduced; deposits may appear behind the cornea; and conjunctival ciliary congestion may be more obvious. 3. When keratitis occurs, the eyes will experience symptoms such as foreign body sensation, photophobia, tearing and decreased vision. 4. Because the virus lurks in the trigeminal ganglion after infection, 1/3 of patients will relapse within 5 years. Repeated episodes will cause corneal opacity and seriously affect vision. 【diagnosis】

1. The diagnosis can be made based on the history of recurrent attacks, prolonged course of disease, corneal lesions and corneal hypoesthesia.

2. Laboratory examinations such as tissue culture, immunofluorescence antibody testing and molecular biology techniques (such as PCR) can assist in diagnosis.
