The degree of local joint inflammation has no obvious relationship with the presence or absence of carditis or heart valve disease.

3. Carditis

It is the most important clinical manifestation, and 65% to 80% of pediatric patients have heart disease. Acute rheumatic carditis is the most common cause of congestive heart failure in children.

4. Skin changes

It can be urticaria, maculopapular rash, erythema multiforme, erythema nodosum and annular erythema. Annular erythema is more common and has diagnostic significance. It is commonly seen on the inner sides of the limbs and trunk. It is a light red ring-shaped blush that is small at first and then quickly expands to the surrounding area. The edges are slightly straightened and the skin inside the ring is normal in color. Sometimes fused into a wreath shape. The red spots appear and disappear from time to time, are not itchy or hard, fade when pressed, and can last for several months.

5. Chorea

It often occurs in children with rheumatic diseases aged 5 to 12 years old, with more females than males. The disease usually occurs 2 to 6 months after streptococcal infection. It is a manifestation of rheumatic fever inflammation invading the central nervous system, including the basal ganglia, cerebral cortex, cerebellum and striatum, and the onset is slow. Clinical manifestations include: when mental disorders occur, there are often restlessness, irritability, decreased comprehension and memory; involuntary movements; muscle weakness and work disorder.

6. Other manifestations

In addition to the typical manifestations mentioned above, rheumatism may occasionally affect other parts of the body and cause rheumatic pleurisy, peritonitis, and vasculitis, which should be paid attention to.
