Only a few patients have an acute history, while most present with a chronic, recurrent course. The pathogens are mostly intestinal pathogens, and routine bacteriological examinations can only detect bacteria in about 20% of patients. Recent studies have shown that urodynamic examinations of patients with prostatitis revealed increased posterior urethral pressure, and the symptoms were alleviated or disappeared after oral administration of α-adrenergic receptor antagonists, suggesting that the smooth muscle capacity of the posterior urethra of the prostate is increased, causing urine turbulence and urine reflux into the prostate. The uric acid in the urine produces chemical stimulation, causing pain. Pathogens enter the prostate through urine, causing infection. Pathological anatomy confirms that prostatitis lesions are generally confined to the peripheral zone, where the glandular ducts open into the posterior urethra in the opposite direction to the vertical line of urine flow, which can easily cause urine reflux. The glandular ducts in the central zone and transitional zone run in the same direction as urine flow and are less susceptible to infection. Recent studies have also found that urate in urine not only irritates the prostate, but can also precipitate into stones, block the gland ducts, and serve as a shelter for bacteria. These findings can illustrate that prostatitis syndrome is actually a common manifestation of multiple diseases, and the clinical manifestations are complex and changeable, which can cause various complications or relieve themselves.

Regardless of the type, chronic prostatitis can present with similar clinical symptoms, collectively known as prostatitis syndrome, including pelvic pain, urinary disturbances and sexual dysfunction. The manifestations of pelvic and sacral pain are extremely complex. The pain is generally located above the pubic bone, lumbosacral region, and perineum. Radiating pain can manifest as pain in the urethra, spermatic cord, testicles, groin, and medial abdomen. It radiates to the abdomen, similar to acute abdomen, and radiates along the urinary tract, similar to renal colic, which often leads to misdiagnosis. Symptoms of urination disturbance include frequent urination, urgency, pain when urinating, difficulty urinating, forked urine stream, dripping after urination, increased number of urinations at night, and milky white discharge from the urethra after urination or during defecation. Occasionally, sexual dysfunction occurs, including decreased libido, premature ejaculation, painful ejaculation, weakened erections, and impotence. The above symptoms may be caused by prostatitis or other diseases of the urogenital tract.
