Treatment of skin ulcers

When the skin ulcers, it is usually caused by not properly treating the wound during the trauma process, because it is particularly easy to leave some bacteria at the wound site at night, these bacteria will cause skin ulcers. If the skin ulcers spontaneously, it is caused by skin diseases, and it must be actively treated. So what are the methods for treating skin ulcers?

1. Cause treatment: For allergic reactions (drug rash), cut off the allergens; for diabetic foot, pay attention to blood sugar control; for physical and chemical injuries, stay away from the pathogenic sources; for occupational diseases, try to eliminate the pathogenic conditions, etc.

2. Supportive treatment: including good local immobilization, elevation of the affected area, good care, etc., pay attention to the balance of water and electrolytes, and provide a diet high in protein, energy and vitamins. Improve the patient's immunity.

3. Drug treatment: anti-allergic drugs, anti-parasitic drugs, antibiotics (anti-infection, wound protection), and drugs that promote ulcer healing, etc.

4. Surgical treatment: Malignant changes or cancers caused by tumors or skin diseases should be treated promptly.

Topical treatments

① Ulcer debridement and dressing change; remove necrotic tissue, scars, old granulation tissue, foreign matter, etc. until relatively healthy tissue with rich blood supply is left. For patients with chronic ulcers with exposed bones and joints who need flap repair, the quality of wound treatment is one of the keys to the success of flap transplantation. Soak and rinse with hydrogen peroxide, saline, and iodine for 5 minutes respectively. Cover with iodine gauze. If local edema is severe, apply 3% hypertonic saline wet compress, or add antibiotics and antifungal drugs based on the results of bacterial and fungal culture and examination.

② Treatment of skin eczema: Fully follow the principles of using topical dermatological drugs: In the acute phase, when there is more exudation, apply 3% boric acid solution for wet compress; when there is less exudation, use glucocorticoid cream; in the chronic phase, use halometasone cream or Bactroban ointment.

③ Local therapy: local irradiation such as helium-neon laser, ultrashort wave, infrared, etc.

④Surgical treatment: For patients with large ulcer area, ulcer that has not healed after long-term dressing changes, or ulcer with exposed bones, joints, tendons or steel plates, surgical treatment should be considered. In this group, 72 cases were treated surgically (47 cases with simple skin grafting and 25 cases with flap repair), and 49 cases were treated non-surgically.

Systemic treatment

The purpose of systemic treatment is to fight inflammation, relieve itching and improve the body's resistance.

① The use of antibacterial or antifungal drugs: based on the results of bacterial and fungal culture and examination, intravenous antibiotics and antifungal drugs are used;

② Use antihistamines, calcium supplements, vitamin C, etc.;

③ Drugs that improve the body's resistance: such as Utilins, etc.;

④Correct anemia or hypoproteinemia.
