TCM treatment of humeral fracture

In case of greater tuberosity fracture complicated by shoulder dislocation, the fracture fragments are reduced together with the reduction of the dislocation. Since surgical neck fractures complicated by greater tuberosity fractures are usually not displaced, no treatment is required; if there is displacement, open reduction and screw fixation can be used. And carry out exercises early to prevent shoulder joint stiffness.

1. Reduction technique: Sit or lie on your back and perform reduction under local anesthesia. The operator stands on the affected side, holds the affected elbow with one hand, and slowly abducts and externally rotates the affected limb. The other hand is placed on the affected shoulder, and the thumb pushes and presses along the supraspinatus and infraspinatus muscles from the inside to the outside. When it reaches under the acromion, the greater tuberosity that has shifted upward and inward is pressed outward and downward to restore it. In case of greater tuberosity fracture combined with anterior shoulder dislocation, the greater tuberosity can usually be reduced on its own after the shoulder dislocation is reduced.

2. Fixation method: After reduction, use an abduction brace to fix the shoulder joint in abduction and external rotation position, and remove the external fixator after 4 weeks.
