TCM treatment of chronic gastritis - acupuncture

The temperature is high during the dog days of summer, and the Yang energy is strong. According to the principle of "treating winter diseases in summer", aromatic and dehumidifying, yang-warming and cold-dispersing drugs can be used for acupoint application or oral administration, which can easily dispel the Yin and cold evils of the spleen and stomach and is very beneficial for the treatment and prognosis of chronic gastritis.

 

1. Acupuncture

Acupuncture method 1:

Use the "Old Ten Needles" recipe. Select Shangwan, Zhongwan, Xiawan, Qihai, Tianshu (double), Neiguan, and Zusanli (double) acupoints. The Zhongwan and Xiawan points are pierced straightly 1-1.5 inches, and the lifting, inserting, twisting and supplementing method is applied, so that the patient feels local soreness, heaviness and a contraction sensation in the stomach. The Neiguan point is pierced vertically for 0.5-1.5 inches, and deep piercing can penetrate the Waiguan point, causing the patient to feel local soreness and swelling, with numbness and electric sensation radiating to the fingertips; or the point can be pierced obliquely upward for 1-2 inches, causing the patient to feel local soreness and swelling, which can spread to the elbows, axillae, chest, etc., using the flat tonification and drainage method or the lifting, inserting, and twisting tonification and drainage method. The Zusanli point is pierced straightly 1-2 inches, so that the patient's soreness, swelling, numbness and electric sensation spread downward to the dorsum of the foot, and sometimes upward to the knee. Use the method of equal tonification and equal drainage or lifting, inserting, tonifying and draining. Insert the needle vertically 1-1.5 inches into the Qihai and Tianshu points, and apply the lifting, inserting, twisting and supplementing method to transmit the needle sensation to the lower abdomen or genitals.

This method has the effects of strengthening the spleen and stomach, regulating qi and relieving pain, and is used to treat chronic gastritis.

Acupuncture method 2:

Use the "New Ten Needles" recipe. Take Zhongwan, Neiguan (double), Zusanli (double), Tianshu (double), Gongsun (double), and Qihai blood. Insert the needle straightly into the Zhongwan point 1-1.5 inches, and apply the lifting, inserting, twisting and supplementing method, so that the patient feels local soreness, heaviness and a contraction in the stomach. Puncture Neiguan vertically for 0.5-1.5 inches, and deep puncture can penetrate the Waiguan point, causing the patient to feel local soreness and swelling, with numbness and electric sensation radiating to the fingertips; or puncture obliquely upward for 1-2 inches, causing the patient to feel local soreness and swelling, which can spread to the elbows, axillae, chest, etc., using the method of flat tonification and flat drainage or the method of lifting, inserting, and twisting tonification and drainage. The Zusanli point is pierced straightly 1-2 inches, making the patient feel soreness, swelling, numbness and electric sensation spreading downward to the dorsum of the foot, and sometimes upward to the knee. Use the method of equal tonification and equal drainage or the method of lifting and inserting tonification and drainage. Insert the needle vertically 1-1.5 inches into the Qihai and Tianshu points, and apply the lifting, inserting, twisting and supplementing methods to transmit the needle sensation to the lower abdomen or genitals. Puncture the Gongsun point vertically 0.5-0.8 inches and apply twisting and tightening method to make the patient feel a local feeling of heaviness and pain.

This method has the effects of strengthening the spleen and stomach, removing blood stasis and relieving pain, and is used to treat chronic gastritis. This prescription can strengthen the spleen's function of assisting transportation and transformation, promote the stomach's function of digesting food and water, inhibit the secretion of gastric acid, protect the gastric mucosa, and promote the repair of the gastric mucosa.

Acupuncture method 3:

Select Zusanli, Neiguan, Zhongwan, Yinlingquan and Pishu points. When needling the Zusanli acupoint, the left hand is placed below the acupoint, and the right hand holds the needle and inserts it slightly upward 1.5-2 inches, using the lifting, inserting, twisting and supplementing method to conduct the needle sensation upward; when needling the Neiguan acupoint, the left hand is placed on the distal end of the acupoint, and the right hand holds the needle and inserts it obliquely 0.5-1 inch toward the elbow joint, using the lifting, inserting, twisting and supplementing method to conduct the needle sensation upward; the Zhongwan acupoint is inserted straightly 1.5-2 inches, and a small amplitude lifting, inserting, twisting and draining method or scraping needle method is used, so that the patient feels local heaviness and a sense of stomach contraction; the Yinlingquan acupoint is inserted straightly 1.5-2 inches, and the lifting, inserting, twisting and draining method is used, so that the patient feels local sourness and electric shock; the Pishu acupoint is inserted obliquely 0.8-1.2 inches toward the spine, so that the patient feels local heaviness. Acupuncture and moxibustion can be performed on the spleen and stomach points on the back or the "three abdomen" points, and cupping can be added after the treatment.

This method has the effects of warming the yang, unblocking the meridians, dispelling cold and relieving pain, and is used to treat pain caused by various types of gastritis.

Acupuncture Method 4:

The main acupoints are Zhongwan, Weishu, Juque, Zusanli, Gongsun and Neiguan; the auxiliary acupoints are Pishu, Fenglong, Zhangmen, Qimen and Yanglingquan. For excess heat syndrome, acupuncture should be used as the main treatment, while for deficiency cold syndrome, moxibustion should be used as the main treatment, with acupuncture as a supplement. Insert the needle vertically 3 inches into the Zhongwan acupoint, and apply the breathing and twisting method of tonification and drainage: insert the needle 1 inch into the Gongsun acupoint, 1.5 inches into the Neiguan acupoint, and insert the needle tip obliquely 5 minutes toward the midline for the Qimen and Zhangmen acupoints. Do not insert into the chest cavity, and twist in and out. The stomach and spleen points are pierced toward the spine with the needle inserted 1 inch in the direction, using the twisting and lifting technique. Acupuncture is performed once a day, and 10 times constitute a course of treatment.

This method has the effects of regulating qi, relieving pain, strengthening the spleen and stomach, and is used to treat chronic gastritis.


2. Electroacupuncture

Electroacupuncture 1:

Select Zusanli, Sanyinjiao, Neiguan and Zhongwan points. Routine local disinfection of acupoints is performed. For Zusanli acupoint, a 5-inch filiform needle is inserted straight at the point of 0.8-1.2 inches, and the lifting, inserting, twisting and supplementing method is applied to transmit the needle sensation upward. For Sanyinjiao acupoint, a 1.5-inch filiform needle is inserted obliquely upward, and the lifting, inserting, twisting and supplementing method is applied to transmit the needle sensation upward. For Neiguan acupoint, a 1.5-inch filiform needle is inserted centripetally and obliquely at the point of 0.5-0.8 inches, and the lifting, inserting, twisting and supplementing method is applied to transmit the needle sensation along the forearm to the stomach. For Zhongwan acupoint, a 2-inch filiform needle is inserted straight at the point of 1.0-1.5 inches, and the lifting, inserting, twisting and supplementing method is applied to make the patient feel local soreness and pain, and a sense of contraction in the stomach. After each acupoint has received qi, connect it to the electroacupuncture device. The current should be within the patient's tolerance. Leave the needle in for 20 minutes, once a day, and 7 times constitute a course of treatment.

This incense stick has the effects of strengthening the spleen and stomach, regulating qi and relieving pain, and is used to treat chronic gastritis.

Electroacupuncture 2:

Select the spleen, stomach and Neiguan points. Routine local disinfection of acupoints is performed. Use a 1.5-inch filiform needle to insert the spleen and stomach points obliquely 0.8-1.2 inches toward the spine. Use a 1.5-inch filiform needle to insert the Neiguan point obliquely 0.5-0.8 inches toward the centripetal point. Apply the lifting, inserting, twisting and supplementing method to transmit the needle sensation along the forearm to the stomach. After each point has received qi, connect the electroacupuncture therapy device to the degree that the patient can tolerate, leave it in for 20 minutes, once a day, 7 times as a course of treatment, and have a second course of treatment after an interval of 3 days.

This method has the effects of strengthening the spleen and stomach, regulating qi and relieving pain, and is used to treat chronic gastritis.


3. Moxibustion

Moxibustion Method 1:

The acupoints are divided into two groups. The first group includes Zhongwan, Neiguan, Liangmen, Qihai, and Zusanli points, and the second group includes Weishu, Pishu, Shenshu, Gongsun, and Neiguan points. Light the moxa stick, insert it into the top hole of the moxibustion device, aim at the selected acupuncture point, and fix the left and right bottom loops with rubber bands so that the body of the device is perpendicular to the skin. Adjust the temperature to a level that is warm or slightly hot but tolerable to the patient; a moderate temperature is ineffective, while a high temperature may burn the skin. For the acupoints on the chest, abdomen and limbs, the patient can be asked to adjust them by himself. The time for moxibustion starts from the first day of the "first", "middle" and "last" of the "fu" every year, once a day, 7 days as a course of treatment, rest for 3 days before the next course of treatment. There are 3 courses of moxibustion treatment in total, and the treatment is generally continued for 3 years.

This method has the effects of warming the middle and strengthening the spleen, harmonizing the stomach and relieving pain, and is used to treat gastritis of spleen and stomach deficiency and cold type.

Moxibustion Method 2:

On the abdomen, select Shangwan, Zhongwan, Xiawan and Liangmen acupoints; on the back and waist, select Pishu, Weishu, Sanjiaoshu and Shenshu points. Choose a large moxibustion box, light 1-3 sections of 6-7 cm long moxa sticks and place them on the iron gauze in the box, aim at the acupoints or moxibustion area, cover the lid and apply moxibustion for 15-30 minutes each time. At the same time, acupuncture the Zhangmen and Zusanli acupoints. If during the moxibustion process, the temperature inside the box is too high and the patient cannot tolerate it, the lid should be partially opened in time (the hot air will rise and the temperature below will drop). If the temperature is still high, you can take off the lid. When covering the lid, be careful not to cover it completely. Leave a certain gap to allow air to circulate and allow the moxa stick to burn fully. The time for moxibustion starts from the first day of the "first", "middle" and "last" dog days every year, once a day, 7 days as a course of treatment, rest for 3 days before the next course of treatment. There are 3 courses of moxibustion treatment in total, and the treatment is generally continued for 3 years.

This method has the effects of warming the middle and strengthening the spleen, harmonizing the stomach and relieving pain, and is used to treat gastritis of the deficiency-cold type of the stomach.

Moxibustion Method 3:

The acupoints are divided into two groups. The first group includes Zhongwan, Liangmen, Qihai, and Zusanli points, and the second group includes Weishu, Pishu, Shenshu, Gongsun, and Neiguan points. Use "white mustard cake" for moxibustion. Grind white mustard seeds into fine powder, mix the powder with cold water, and make it into a medicine cake with a diameter of 2.5-3.0 cm and a thickness of 0.3-0.4 cm. Place the medicine cake on the selected acupuncture points or parts, place the moxa cone on the medicine cake and light it for moxibustion. Moxibustion was applied to the Zusanli point 9 times, the Pishu and Weishu points 7 times each, the Zhongwan point 7 times, and the Liangmen point 7 times. The time for moxibustion starts from the first day of the "first", "middle" and "last" of the "fu" every year, once a day, 7 days as a course of treatment, a total of 3 courses of treatment, generally continuous moxibustion for 3 years.

This method has the effects of warming the middle and stomach, dispelling cold and relieving pain, and is used to treat gastritis of spleen and stomach deficiency and cold type.

Moxibustion Method 4:

Take Zhongwan, Liangmen, Pishu, Neiguan, Zusanli and Guanyuan points. When using the "moxibustion through ginger" method, the patient should lie prone when moxibustion is applied to the spleen point; when moxibustion is applied to the Zusanli, Guanyuan, Neiguan, Liangmen, and Zhongwan points, the patient should lie down. Cut fresh ginger into 0.3-0.4 cm thick slices, poke a few small holes on it with a needle, then place the ginger on the selected acupuncture points, place a 1.5 cm x 2 cm moxa cone on it, and light it with an incense stick. When the patient feels burning, lift the ginger slices and the moxa cone slightly upwards, or remove the moxa cone immediately and replace it with a new one to continue moxibustion. The time for moxibustion starts from the first day of the "first", "middle" and "last" of the "fu" every year. Each acupoint is moxibustioned 3-9 times, and the moxa cone is changed but the ginger slice is not. Once a day, 7 days as a course of treatment, a total of 3 courses of treatment, generally continue moxibustion for 3 years.

This method has the effects of warming the middle and dispelling cold, harmonizing the stomach and relieving pain, and is used to treat gastritis of the deficiency-cold type of the stomach.


4. Auricular acupuncture

Ear acupuncture 1:

Take the ear acupoints spleen, stomach, mouth, subcortical, sympathetic, and Shenmen. Choose a 0.5-inch short-handled filiform needle for the treatment. Perform routine local disinfection on the ear acupoints, fix the ear with the left hand, and insert the needle with the right hand. The depth of needle insertion should be enough to penetrate the soft tissue without going through the opposite skin. The needle should be left in place for 20-30 minutes. During this period, the needle can be twisted at intervals. Or "thumbnail-type" intradermal needle (i.e., push needle), perform routine local disinfection on the ear acupoint, hold the needle handle with tweezers, aim at the selected acupoint, and insert it vertically, so that the ring-shaped needle handle remains flat on the skin, fix it with adhesive tape, and leave the needle for 2-3 days. During the needling period, press the needle site with the thumb and index finger 2-3 times a day to strengthen the needle sensation and enhance the therapeutic effect. Or take the seeds of Vaccaria and perform routine local disinfection on the ear acupoints. Place the seeds of Vaccaria in the center of an 8mm x 8mm adhesive tape, apply it to the selected treatment point, press with the thumb and index finger, and knead until the ear acupoint becomes locally hot, red, and painful. Leave it on for 3-5 days, pressing 2-3 times a day.

This method has the effects of warming the yang and invigorating the qi, dispelling cold and relieving pain, and is used to treat gastritis caused by cold evil invading the stomach.

Ear acupuncture 2:

Select the ear points of liver, stomach, sympathetic, boat center, and spleen. Select 0.5-inch short-handled filiform needles for acupuncture, perform routine local disinfection on the ear acupoints, fix the ear with the left hand, and insert the needle with the right hand. The depth of needle insertion should be enough to penetrate the cartilage but not the skin on the opposite side. The needle should be left in place for 20-30 minutes, during which time the needle may be twisted at intervals. Or use tweezers to hold the intradermal needle and pierce the selected acupuncture point vertically, fix it with adhesive tape, and leave the needle for 2-3 days. During the needling period, press the needle site with the thumb and index finger 2-3 times a day to strengthen the needle sensation and enhance the therapeutic effect. Or place the Chinese medicine Vicia fructus in the center of an 8mm x 8mm adhesive tape, apply it to the selected treatment point, press with the thumb and index finger, and knead until the ear point becomes locally hot, red, and painful. Press 2-3 times a day.

This method has the effect of clearing heat and soothing the stomach, and is used to treat gastritis caused by heat accumulation in the liver and stomach.

5. Acupoint injection

Acupoint injection method 1:

Use a 10 ml syringe to draw 10 ml of "Astragalus Injection", select Pishu, Zhongwan, Sanjiaoshu, and Neiguan acupoints, perform routine local disinfection, insert the acupoints, and slightly lift and insert until the patient feels a sore and swollen sensation in the acupoints and no blood returns when aspirated, then inject the medicine, 2.5 ml for each acupoint. Inject one side once every other day, and then inject the other side the next time. 6 times constitute a course of treatment. This method is used to treat chronic gastritis.

Acupoint injection method 2:

Use a 10 ml syringe to draw 10 ml of "Compound Danshen Injection", select Pishu, Neiguan, Gongsun, and Zusanli acupoints, perform routine local disinfection, insert the acupoints, and slightly lift and insert until the patient feels a sore and swollen sensation in the acupoints and no blood returns when aspirated, then inject the medicine, 2.5 ml for each acupoint. Inject one side once every other day, and then inject the other side the next time. 6 times constitute a course of treatment. This method is used to treat chronic gastritis.

Acupoint injection method 3:

Use a 10 ml syringe to draw 10 ml of "Danggui Injection", select the spleen point, stomach point, triple burner point, and Zhongwan point, perform routine local disinfection, insert the needle into the point, and slightly lift and insert until the patient feels a sore and swollen sensation in the point and no blood returns when aspirated, then inject the medicine, 1.5 ml for each point. Inject one side every other day. This method is used to treat chronic gastritis.

(6) Acupuncture thread embedding method: The spleen point, stomach point, liver point, Zhongwan point and Zusanli point are selected. Under local anesthesia, make a 1 cm incision in the skin and subcutaneous tissue, separate the muscle layer, and search for sensitive points deep inside. When sensory phenomena occur, specific induction techniques are applied to expand the base surface, and appropriate gut (sheep gut soaked in 75% alcohol for 1 hour) is buried depending on the condition and physical condition, and at a certain angle to the incision. After burying, rinse and disinfect, suture with one stitch, cover with sterile gauze, and remove the stitches after 7 days. Avoid damaging thicker blood vessels and nerves, and do not use anti-inflammatory analgesics within 6 weeks. This method is used to treat chronic gastritis.
