TCM treatment of tinnitus, TCM dialectical treatment of tinnitus

Simply put, tinnitus is a problem with the auditory system. Without the stimulation of external sounds, there is a buzzing noise or other strange sounds in the ears. Tinnitus patients often also have other complications such as dizziness. The main method of TCM in treating tinnitus is dialectical treatment.

1. Liver fire disturbance type

1: This type of patients often have sudden onset of symptoms, severe symptoms, and loud tinnitus, which is closely related to changes in emotions and often occurs or worsens after depression and anger. It is often accompanied by symptoms of liver fire rising, such as bitter taste in the mouth, irritability, dizziness, headache, impatience, etc., with a red tongue, yellow coating, and a stringy and rapid pulse. This type is caused by liver fire disturbing the clear orifices. The treatment should be to clear the liver, drain heat and open the orifices. The prescription should be Longdan Xiegan Decoction with modifications. Commonly used medicines include: dandelion, scutellaria, gardenia, angelica, oriental water plantain, rehmannia, akebia, bupleurum, calamus, vitex rotundifolia, and magnetite.

2. Phlegm and turbidity type

1: This type of disease usually has a long course and is characterized by tinnitus, dizziness, heaviness, and stuffiness in the head, as well as an obvious feeling of fullness and blockage in the ears. It may be accompanied by chest tightness, poor appetite, a fat tongue with teeth marks on the edges, thick and greasy tongue coating, and a wiry and slippery pulse. This type of syndrome is caused by phlegm and turbidity blocking the clear orifices. The treatment should be to resolve phlegm and turbidity, harmonize the stomach and open the orifices. Erchen Decoction with modifications is often used. Commonly used medicines include: Pinellia, dried tangerine peel, Poria, Bombyx batryticatus, Acorus calamus, raw coix seed, and Angelica dahurica. You can appropriately choose 1 to 2 blood-activating and blood-stasis-removing medicines, such as Salvia miltiorrhiza, peach kernel, safflower, and Angelica sinensis root, to help open the orifices by removing blood stasis, which is also in line with the theory of traditional Chinese medicine that long-term illness is often caused by blood stasis. If there are signs of phlegm and turbidity turning into heat, add Scutellaria baicalensis, Coptis chinensis, Trichosanthes kirilowii, and Citrus aurantium.

3. Weak spleen and stomach type

1: Patients often show poor spirits, fatigue, dizziness, worsening symptoms after fatigue, poor appetite, pale tongue, thin white or thick tongue coating, and weak pulse. The treatment is to strengthen the spleen and replenish qi, raise yang and open the orifices. You can choose prescriptions such as Buzhong Yiqi Decoction, Shenling Baizhu Powder, and Bazhen Decoction. Commonly used medicines include: Codonopsis pilosula, Astragalus membranaceus, Atractylodes macrocephala, Cimicifuga heracleifolia, Pueraria lobata, Bupleurum chinense, Tangerine peel, Pinellia ternata, Angelica sinensis, and Acorus calamus.

4. Liver and kidney deficiency type

1: This type of disease has a long course and, according to clinical conditions, mostly occurs in the elderly. The sound in the ear is like the chirping of cicadas, which is generally not very loud. It may be accompanied by symptoms of kidney deficiency such as soreness of waist and knees, dizziness, dry eyes, etc. The accompanying symptoms are often not obvious. The tongue is red, with little coating, and the pulse is thin. Tinnitus in the elderly without other concomitant symptoms can usually be classified into this type. The treatment is mainly focused on tonifying the kidney and replenishing essence, nourishing yin and suppressing yang. The prescription of choice is Erlong Zuoci Pills. Commonly used medicines include raw and cooked rehmannia, cornus officinalis, yam, calamus, Achyranthes bidentata, Cuscuta australis, raspberry, cicada shell, magnet, Shenqu, Pueraria root, and Bupleurum.

2: Clinically, tinnitus is often seen accompanied by relatively obvious symptoms of heart fire inflammation, such as irritability, impatience, insomnia, mouth and tongue ulcers, red tongue tip, and thin and rapid pulse. It is more common among young people, but may also occur in some elderly people. This is caused by liver and kidney deficiency, water cannot support fire, heart fire rises and disturbs the clear orifices. The treatment should focus on clearing heart fire, and secondly nourishing kidney yin. The prescription of choice is Daochi San with modifications. Commonly used medicines include: raw rehmannia, scutellaria baicalensis, coptis chinensis, loblolly leaf, akebia, junci, dodder seed, ophiopogon japonicus, rubus idaeus, and jujube seed.

5. Wind-heat invasion type

1: This type of patient has an acute onset, but the symptoms are usually not very severe. The tinnitus has a low pitch, and there is a feeling of fullness and blockage in the ear, and the patient often has increased self-talk. It is often accompanied by lung meridian symptoms such as nasal congestion, runny nose, and cough. The Western medical diagnosis is mostly acute secretory otitis media. The tympanic membrane is seen to be congested and sunken, the tongue is pale red, the tongue coating is thin white, and the pulse is floating and rapid. For the treatment of one syndrome, we should adopt Liu Yuansu's view of "deafness by treating the lungs", and treat it from the lungs, using the methods of dispersing wind, promoting lung function, and opening the orifices. Prescriptions such as Yinqiao Powder can be used. Commonly used medicines include double flower, forsythia, platycodon, apricot, schizonepeta, saposhnikovia, cicada shell, and calamus. If there is excessive phlegm, add trichosanthes kirilowii, and if there is nasal congestion, add magnolia, Xanthium sibiricum, and Angelica dahurica.

6: Tinnitus affects the auditory system

It is the result of different pathological changes of multiple diseases, with complex causes and unclear mechanisms. The main manifestation is the absence of corresponding external sound sources or electrical stimulation, but the subjective sensation of sound in the ear or brain. Clinically, it is a concomitant symptom of many diseases.
