The early symptoms of pyelonephritis are so harmful

Nephritis is a common disease with many early symptoms. The most common ones are sudden unilateral or bilateral low back pain, often accompanied by high fever, chills, nausea, vomiting, sepsis, hypotension, etc. Urinary tract infections in the elderly or weak may have no local urinary tract symptoms but may manifest as fever or even hypotension.

One of the early symptoms of pyelonephritis

1. Sudden onset of unilateral or bilateral low back pain, often accompanied by high fever, chills, nausea, vomiting, sepsis, and hypotension.

2. It may be accompanied by cystitis and difficulty urinating.

3. Tenderness (pain) at the costal angle of the spine.

4. Microscopic examination of urine shows white (pus) blood cells, red blood cells, and epithelial cells. Leukocyte casts may be seen. Urine protein was negative or trace.

5. Urinary tract infection in the elderly or weak may not have local urinary tract symptoms but may manifest as fever or even hypotension.

6. Some people may have no symptoms at all but have significant bacteriuria. Occasionally, it may be accompanied by acute renal papillary necrosis or perinephric abscess.

Early symptoms of pyelonephritis

1. Urinary tract irritation symptoms

Pyelonephritis is mostly caused by ascending infection, so it is often accompanied by cystitis, and patients experience urinary tract irritation symptoms such as frequent urination, urgency, and pain when urinating. The urine is turbid and there is occasional hematuria.

2. Systemic symptoms

It includes chills and fever, with body temperature reaching 39-40℃, fatigue, loss of appetite, nausea, vomiting, or abdominal pain, and increased neutrophil count in the blood, which can easily be misdiagnosed as acute abdomen such as acute cholecystitis or acute appendicitis.

3. Local signs

Pain in one or both kidney areas, with percussion pain and tenderness in the costal area. In addition, there is deep tenderness at the costal lumbar point (the intersection of the outer edge of the psoas major muscle and the twelfth rib) and the upper ureter point (the outer edge of the rectus abdominis muscle at the level of the navel). Urinary tract infection caused by urinary catheterization or complicated by urinary tract obstruction is mostly acute pyelonephritis, which can often present as a fulminant process.

course), sepsis occurs. This type of sepsis is called urosepsis. It is prone to endotoxic shock, has a serious condition, and is more common in the elderly.

Patients with existing diabetes, analgesic nephropathy or urinary tract obstruction who are complicated by acute pyelonephritis may develop acute renal papillary necrosis. In addition to severe systemic symptoms like sepsis and hematuria and pyuria, patients sometimes suffer from ureteral colic due to the shedding of necrotic papillae. Some patients also experience oliguria or urinary retention and acute renal failure.

Early symptoms of pyelonephritis

This disease can occur at all ages, but is most common in women of childbearing age. It has an acute onset and the main symptoms are as follows.

High fever and chills, body temperature is mostly between 38 and 39 degrees Celsius, and can be as high as 40 degrees Celsius. The fever types vary, generally presenting as a remittent type, but may also be intermittent or prolonged. Accompanied by headache, body aches, and sweating when the fever subsides. Patients have low back pain, mostly dull pain or soreness of varying degrees. A few have abdominal colic that radiates along the ureter toward the bladder. During physical examination, there is tenderness at the upper ureteral point (the intersection of the outer edge of the rectus abdominis and the umbilical line) or the costal lumbar point (the intersection of the outer edge of the psoas major and the twelfth rib), and renal percussion pain is positive. Patients often have symptoms of bladder irritation such as frequent urination, urgency, and pain when urinating. The urinary system symptoms in children are often not obvious. In addition to systemic symptoms such as high fever, they often have convulsions and seizures at the onset of the disease. There may be loss of appetite, nausea, vomiting, and some patients may have pain in the upper or middle abdomen.
