Interventional treatment for epistaxis

Interventional treatment for epistaxis

Nosebleed is a relatively common disease with many causes. The most common one is local lesions in the nasal cavity. Of course, some systemic diseases may also cause nosebleeds. Nosebleeds should be treated in time, especially for severe nosebleeds. Interventional methods can be used for treatment. Endoscopic microwave coagulation can be used to treat nosebleeds, and the effect is good.

Self-First Aid Methods

1. Do not lie on your back when you have a nosebleed. Try to spit out the blood that has flowed into your mouth to prevent the blood from being accidentally inhaled into the respiratory tract and causing suffocation.

2. Avoid stuffing it with paper rolls or cotton.

3. The best way is to apply pressure to stop bleeding. Use your fingers to squeeze the nose wings towards the septum, and apply a cold towel to the nose. Generally, apply pressure for about 5 to 10 minutes and the bleeding will stop. If the bleeding still does not stop, go to the hospital immediately.

treat

Treatment should be given according to the condition and cause. For small amounts of bleeding, ephedrine nasal drops can be applied topically. Or apply chloramphenicol eye ointment to the mucosal erosion. When the above methods are ineffective, nasal packing is mostly used. Vaseline gauze, expandable sponges, and hemostatic gauze are often used to pack the anterior nostril. When there is bleeding from the posterior nostril, the posterior nasal packing method is sometimes used. This causes great pain to the patient and may cause abrasions and erosions of the nasal mucosa, affecting the patient's eating and sleeping. In the elderly and patients with myocardial disease, their oxygen saturation may decrease at night, which may induce myocardial infarction and cerebrovascular accident.

Endoscopic microwave coagulation for the treatment of epistaxis

It uses the clear field of view and illumination of the nasal endoscope to observe hidden and difficult-to-detect bleeding sites such as the olfactory groove, the sphenopalatine artery blood supply area at the posterior end of the middle turbinate, the posterior segment of the nasal septum, the posterior end of the inferior turbinate, the Woodruff venous plexus and the top of the nasopharynx. Microwaves are a high-frequency electromagnetic wave that can instantly generate high temperatures in tissues, achieving the purpose of coagulation and hemostasis.

Microwave probes of different shapes are used to accurately cauterize and stop bleeding at the bleeding site. The operation is flexible and the hemostasis is reliable. It can minimize the damage to the surrounding normal tissues. Compared with laser, it has the characteristics of no carbonization and no smoke formation. The most important thing is that there is no need for nasal packing after surgery, which reduces the pain of patients. It is a rapid, accurate, effective and safe method of stopping bleeding.

For small amounts of repeated bleeding, especially blood in the retracted nasal mucus, fiberoptic snuffoscope examination must be performed to rule out nasopharyngeal carcinoma and other malignant tumors.
