Characteristics of Medulloblastoma

(1) Pathology of Medulloblastoma Medulloblastoma is soft and friable, with barely discernible borders. The cut surface is gray-red, and larger tumors often have central necrosis. The vast majority of tumors are located in the cerebellar vermis. It protrudes into the fourth ventricle and often invades the floor of the fourth ventricle. From the fourth ventricle, the tumor blocks the aqueduct and the outlet of the fourth ventricle, causing hydrocephalus. Medulloblasts have a tendency to diffusely metastasize along the subarachnoid space. The pia mater adjacent to the tumor is often infiltrated, forming a layer of milky white colloid tissue on the surface of the brain. The tumor may metastasize to the spinal canal and the surface of the brain, or spread along the ventricles to the floor of the third ventricle.

(2) Clinical manifestations of medulloblastoma: The tumor is highly malignant, grows rapidly, and has a short course. The main manifestations are increased intracranial pressure and cerebellar symptoms. The frequencies of symptoms were: vomiting 82%, headache 79%, gait instability and ataxia 56%, diplopia 34%, and decreased visual acuity 27%. The frequency of signs was: papilledema 75%, nystagmus 56%. Ataxia and difficulty standing with eyes closed were found in 56% and abducens nerve palsy in 47%. When the tumor invades the brainstem, cranial nerve symptoms and manifestations of pyramidal tract damage may occur. Forced head position and neck resistance may also occur.
