What to do if your toenails separate from the flesh

Treatment methods for the cure of onychomycosis

1. Surgical nail removal: Suitable for single nails. The affected nail will be removed under local anesthesia. This method causes large wounds, bleeding, and is prone to infection. The operation generally causes more pain and is prone to recurrence of infection, so it is rarely used in clinical practice. Patients with heart disease, high blood pressure, diabetes, etc. are not suitable for surgical treatment. 2. Oral medication: suitable for multiple nails. For example, take 250 mg of terbinafine tablets orally daily for 6 to 12 weeks; take itraconazole 400 mg/day for 7 days and rest for 21 days as a course of treatment, and continue for 3 to 6 courses; take fluconazole 150 mg 1 to 2 times a week for more than 4 months. It can cure more than 80% of onychomycosis and onychomycosis. However, because this type of drug must reach the deck where the fungus is parasitic to exert its antibacterial effect, the dosage is large and the medication time is long, adverse drug reactions should be monitored regularly.

3. External therapy: Depending on the different medicines, the treatment methods are different, mainly local application of medicines and packing and cutting treatments. (1) Use a nail file (non-professionals should avoid using a razor blade to avoid bleeding and infection) to grind the irregular and damaged nails into thinner pieces. The best grinding cycle is once every two days. (2) Apply 30% glacial acetic acid or soak the damaged nails in 10% glacial acetic acid once a day for more than 3 to 6 months. The effect will be better if the diseased nail is thinned before applying the medicine. Before applying nail polish, be sure to protect the skin around the nails with a mild ointment; (3) Use antifungal topical medications such as terbinafine tincture, amorolfine nail polish, and ciclopirox nail polish for local application.

Early symptoms of onychomycosis

1. Subungual onychomycosis: It often starts from the sides or ends of the nail plate, usually with mild paronychia at first, and then gradually becomes chronic or gradually disappears. Paronychia can cause pits or grooves on the nail surface, which may persist or gradually involve the nail root. Once the nail plate becomes infected, it may develop cracks, become brittle, or thicken and appear brown or black. This type is common. Due to the deposition of keratin and debris under the nail, the nail becomes loose and becomes cloudy and thickened. 2. Fungal leukonychia (superficial white onychomycosis): There are one or more small turbid areas on the surface of the nail plate with irregular shapes. They can gradually spread to the entire nail plate, causing the nail surface to soften and sink. There are no symptoms, no paronychia, and desquamation is often seen in the skin of the nail bed folds. 3. Onychomycosis caused by Candida albicans: This type of onychomycosis mainly occurs in housewives, cooks and people who often come into contact with water. It is often accompanied by paronychia, which starts from the nail folds on both sides and may cause skin redness, swelling, pus accumulation and tenderness. The nearby nail becomes dark, rises, and separates from the nail bed underneath, and then the entire nail plate is affected.
