How to distinguish between false strabismus and true strabismus

Pseudo-strabismus is often confused with true strabismus, and it is not easy to tell it is pseudo-strabismus at first glance. Even experienced clinicians have to conduct repeated examinations to make the correct judgment. Pseudoexotropia is mainly caused by excessive pupil distance, which will cause a sense of exotropia. Normal eye position is considered pseudostrabismus and does not require treatment.

Causes of Pseudoesotropia

1Intermittent esotropia in infants. It mainly occurs in infants within 4 months of birth. When the two eyes look at close objects, they converge and develop intermittent esotropia. After 4-6 months, when the eyes begin to adjust, the esotropia may disappear on its own.

2. The pupil distance is too small. When the distance between the two eye sockets and the pupil is too small, it will give people the feeling of esotropia.

3. Inner fat skin. This condition is most easily mistaken for esotropia and is also the most common type of pseudostrabismus. This is because the epicanthus covers the inner part of the eye, and in severe cases it can cover part of the sclera, so that the sclera on the nasal side is less exposed than the temporal side. In addition, the baby's nose root is too wide, giving the baby a feeling of esotropia (cross-eyed) from the appearance. When the child's eyeballs turn left or right, the eye that turns inward becomes more obvious.

Identification

The method to identify esotropia is to check the eye position by alternately covering one eye. Or pinch the skin at the root of the child's nose to expose more of the sclera on the nasal side, and the "esotropia" will disappear. Another method is to use corneal reflection, which is to shine a flashlight between the child's eyes to see if the light is in the middle of the cornea.

Preventive health care methods for strabismus and amblyopia

1. Occlusion therapy should be gradually removed after vision returns to normal. Start by opening it for 2 hours a day. After 1 month, when the therapeutic effect is consolidated, extend the opening time to 4 hours a day, then to 6 hours, 8 hours, until it is open all day or change from full coverage to half coverage to consolidate the therapeutic effect. During the period of consolidating the therapeutic effect, do not relax the fine work.

2 If you find that the vision of the amblyopic eye has decreased, you can cover the healthy eye again and the amblyopic eye can still improve to its original level.

3. After vision returns to normal, a follow-up check-up is required once a month for the first 6 months, and then once every 3 months and 6 months, until complete recovery is achieved after 3 years of follow-up.

4. In addition to vision enhancement therapy for strabismus and amblyopia, it is also crucial to work hard to train the single vision function and image integration ability of both eyes. If vision loss is found, afterimage therapy should be resumed. Afterimage therapy should not be stopped suddenly, but the frequency should be gradually reduced and the interval should be prolonged, and it should be stopped slowly. After stopping, the amblyopic eye should be used frequently to watch movies, TV, write small characters, do fine work, or play drawing games on a turntable with black and white lines. Through these simple and easy methods, the macular function can be stimulated to prevent regression.
