Vascular puncture is the first step in completing interventional angiography and interventional vascular treatment, and is also a necessary guarantee for Seldinger technology. Puncture needles generally include ordinary puncture needles and special puncture needles. Ordinary puncture needles are divided into single-wall puncture needles and cannula puncture needles. The needle is generally 7.0 cm long, with a needle diameter of 18 to 19 G, and can be inserted with a 0.015 in to 0.038 in guide wire. The single-wall puncture needle is made of a single component of stainless steel with a beveled needle tip and is mainly used for puncturing the anterior wall of the blood vessel. The cannula puncture needle consists of an outer cannula and a needle core. Currently, the commonly used needle cores are mostly hollow needles with sharp needle tips that protrude outside the outer cannula. The outer sleeve is made of plastic. The puncture needle is inserted into the blood vessel lumen, the needle core is removed, the outer sleeve is left in the lumen, and then the guide wire is inserted along the lumen of the outer sleeve. It is commonly used for puncture of common sites of peripheral arteries and veins. There is also a micropuncture cannula needle, which is mainly used for puncture of smaller arteries such as the brachial artery and radial artery. The needle diameter is 21G, and a 0.018in guide wire can pass through the needle cavity. Special puncture needles mainly include the atrial septal puncture needle (Brockenbrough puncture needle) and the TIPSS special puncture needle set, both of which are puncture devices with cannula. Among them, the Brockenbrough puncture needle is about 70cm long, with a 4cm long bend at the front end and a direction indicator plate at the tail of the needle that is consistent with the direction of the needle tip at the front end. The needle shaft is 18G and the needle tip is reduced to 21G. It is mainly used to open the occluded segments of the inferior vena cava and hepatic vein in Budd-Chiari syndrome. The TIPSS special puncture needle set consists of a long puncture needle, an outer sleeve, a metal guide puncture needle and sleeve, and a 10F long sheath. There are two designs: German and American. The front end of the metal guide puncture needle is bent at two angles, 30° and 60°, so the angle can be easily adjusted during the operation to complete the puncture. It is mainly used for portal vein puncture during TIPSS and for opening of the occluded segment of the inferior vena cava in Budd-Chiari syndrome. |
