Pulmonary tuberculosis is a chronic infectious disease caused by Mycobacterium tuberculosis. Any organ in the body may be affected, but pulmonary tuberculosis is the most common. It belongs to the categories of "pulmonary tuberculosis", "tuberculosis" and "bone steaming" in Chinese medicine. The main clinical manifestations are hot flashes, night sweats and dry cough.

The lungs control qi, regulate breathing, and are the source of water. The body's fluid metabolism is carried out through the lungs, where the clear fluid is distributed throughout the body to nourish it. The turbid fluid is also carried to the bladder through a descending function to become urine and be excreted from the body. Therefore, the lung's functions of spreading and descending complement each other and together complete the task of controlling the metabolism of qi and water. If tuberculosis nematodes invade the lungs, reside in the lungs, and corrode the lung system, it will inevitably affect the lung's metabolism of qi and water, and cause a variety of pathological changes.

(1) Turbid water and blood stasis combine to form tuberculosis. When tuberculosis mites invade the lungs, the lungs are damaged, Qi cannot circulate, blood circulation is blocked, and Qi stagnates and blood stasis occurs. The circulation of water depends on the lung's Qi transformation, dispersion and descent. When tuberculosis mites invade the lungs, the lung Qi is damaged, and the Qi transformation and circulation of water are blocked, causing it to accumulate and become turbid water. Turbid water and blood stasis combine to form tuberculosis. The basic pathological changes of pulmonary tuberculosis mentioned in modern medicine include exudative lesions, proliferative lesions, dry lesions, and hilar lymph node tuberculosis, all of which fall within the scope of this theory.

Turbidity and blood stasis turn into heat, secretly depleting the lung yin. Turbid water and blood stasis are both pathological products caused by organ deficiency or invasion of evil toxins. It stays in the body, infiltrates the internal organs, skin, and bones, turns into heat, and secretly consumes the lung yin, causing symptoms such as bone steaming, hot flashes, red cheeks, night sweats, dry coughs, palpitations, insomnia, nightmares, spermatorrhea in men, irregular menstruation in women, red tongue with little coating, and deep, thin, weak pulse, etc., which are signs of yin deficiency and hyperactivity of fire. This is the reason why Zhu Danxi of the Yuan Dynasty said that "tuberculosis is mainly caused by yin deficiency".

(3) Burns caused by fire or metal, cough, hemoptysis, and hoarseness. Yin deficiency and excessive fire, the virtual fire burns inside the body, burns the lung metal, causes loss of clearing and purging, dry cough without sputum or pus-filled sputum; fire damages the lung meridian, the blood vessels are damaged, there is blood in the sputum or hemoptysis; the virtual fire rises, causing sore throat, hoarseness, and speaking like a duck. If the disease is not cured for a long time and the lung tissue is severely damaged, it may be accompanied by lung atrophy, emphysema, difficulty walking, cyanosis and other lung function disorders, and the condition will worsen.

(4) Yin damages Yang, resulting in deficiency of both Qi and Yin. Pulmonary tuberculosis is dominated by Yin. With prolonged Yin deficiency, Yin cannot restrain Yang, and Yang becomes deficient in the end, resulting in Qi and Yin deficiency, which is manifested by low fever and sweating, cough, shortness of breath, fatigue, thin body, pale complexion, poor appetite, loose stools, clear and long urine, pale tongue with little coating, and deep, thin and weak pulse.
