What are the signs of purpura recurrence?

Omen 1: The patient develops needle-shaped "red spots" on the extensor side of the skin of the limbs, which do not fade when pressed, especially on the lower limbs, around the ankle joints and buttocks. The "red spots" on the skin are generally distributed symmetrically and do not merge with each other, and the "red spots" are prone to recurrence. Omen 2: There is a problem with the digestive system and the patient will vomit. About half of the children have positive fecal occult blood test, some may have bloody stools, or even vomit blood. Clinically, it is easy to misdiagnose as surgical acute abdomen. Differentiate deficiency or excess based on the onset, course of disease, color of purpura, etc. Those with acute onset, short course, and brighter purpura are mostly positive; those with slow onset, recurrent symptoms, lingering course, and lighter purpura are mostly negative. Those with wind-heat exterior symptoms such as fever, aversion to wind, and red throat are suffering from wind-heat damaging the collaterals; those with restlessness, thirst, constipation, and red urine, or even nosebleeds, bleeding from the teeth, blood in the stool, and blood in the urine are suffering from blood heat; those with fatigue, dizziness, palpitations, and loss of appetite are suffering from qi failure to retain blood; those with low fever, night sweats, hot hands and feet, and a red tongue with little fluid are suffering from yin deficiency and fire inflammation.

Pay attention to judging the severity of the disease. The severity of the disease is determined by the amount of bleeding and whether it is accompanied by kidney damage or intracranial hemorrhage. Those with less bleeding are considered mild cases; severe bleeding accompanied by large amounts of blood in the stool, hematuria, obvious proteinuria, or headache, coma, convulsions, etc. are all severe cases.

Treatment principles

The treatment of this disease is mainly based on clearing heat and cooling blood for excess syndrome; and mainly based on invigorating qi, arresting blood, nourishing yin and reducing fire for deficiency syndrome. In clinical practice, we must pay attention to the mutual transformation or simultaneous occurrence of syndrome types. During treatment, it is advisable to distinguish the primary from the secondary and take a comprehensive approach.
