【Treatment method】: Regulate Qi and relieve depression, resolve phlegm and eliminate goiter.

[Prescription]: Sihai Shuyu Pills with modifications. For those with chest tightness and flank pain, add bupleurum, curcuma, and cyperus to regulate qi and relieve depression. If there is discomfort in the throat and neck, add platycodon, burdock, iris, oroxylum bungeanum and other products that can relieve throat and swelling.

2. Phlegm and blood stasis

[Main symptoms]: A lump appears in the front of the neck, which is hard or has nodules when pressed. The lump does not disappear for a long time, and there is chest tightness, poor appetite, thin white or greasy tongue coating, and a stringy or astringent pulse.

[Pathology]: Qi stagnation, body fluid condenses into phlegm, phlegm and Qi are blocked, and over time, blood circulation is blocked and blood vessels become stagnant. Qi, phlegm and blood stasis accumulate in the front of the neck, so the goiter is hard or has nodules and does not disappear for a long time. Qi stagnation and phlegm obstruction cause the spleen to fail to function properly, resulting in chest tightness and poor appetite. White and greasy tongue coating and stringy or wiry pulse are signs of phlegm and blood stasis.

[Treatment method]: Regulate Qi and activate blood circulation, resolve phlegm and eliminate goiter.

[Prescription]: Modified Haizao Yuhu Decoction. If the lumps are hard or have nodules, add Dioscorea bulbifera, Trillium, Curcuma zedoaria, Beehive, Paniculata, Salvia miltiorrhiza, etc. to enhance the effects of promoting blood circulation, softening hard lumps, eliminating goiters and dispersing nodules. For those who have long-term depression and transform into fire, with symptoms such as fever, red tongue, yellow tongue coating, and rapid pulse, add Prunella Vulgaris, Cortex Moutan, and Scrophularia ningpoensis to clear away heat and purge fire. For those with poor appetite and loose stools, add Atractylodes macrocephala, Poria cocos, and Chinese yam to strengthen the spleen and replenish qi.

3. Excessive liver fire

[Main symptoms]: Mild or moderate swelling in the front of the neck, the lump is soft. Symptoms include irritability, spontaneous sweating, excessive appetite, quick hunger, irritability, hot face, bitter taste in the mouth, etc. Excessive liver fire leads to intense stomach heat, so the food is overripe and you feel hungry easily after eating too much. The heat forces the body fluid to leak out, causing spontaneous sweating. Liver fire rises; wind-yang becomes excessive inside the body, causing bulging eyes and trembling fingers. A red tongue, yellow coating and a stringy pulse are signs of hyperactivity of liver fire.

【Therapeutic Method】: Clear the liver and relieve heat

[Prescription]: Gardenia Liver-Clearing Decoction combined with Algae Medicinal Powder with modifications. For those who are irritable, easily angered, and have a rapid and stringy pulse, add Prunella Vulgaris and Gentiana scabra to clear the liver and reduce fire. If Fengyang is excessive and fingers are trembling, add Semen Cassiae, Uncaria Rhynchophylla, Tribulus Terrestris, and Oyster to calm the liver and extinguish wind. If you eat too much and get hungry easily, add gypsum and Anemarrhena asphodeloides to clear stomach heat.

4. Heart and liver yin deficiency

[Main symptoms]: The goiter may be large or small, soft in texture, with a slow onset, palpitations, restlessness, insomnia, sweating, trembling fingers, dry eyes, dizziness, fatigue, red tongue, trembling tongue, and a stringy and fine pulse.

[Pathology]: Stagnation of phlegm and qi leads to goiter. Fire depression damages Yin, causing deficiency of heart Yin and loss of nourishment to the heart, resulting in palpitations, restlessness, irritability and insomnia. Deficiency of liver yin and lack of nourishment to the tendons and veins will cause fatigue and weakness. The liver opens up to the eyes. If the eyes are not nourished, they will become dry and stringy. When the liver yin is deficient and internal wind moves, the fingers and tongue will tremble. A red tongue and a thin, rapid and stringy pulse are signs of yin deficiency and heat.

[Treatment method]: Nourish Yin essence, calm the mind and soften the liver.

[Prescription]: Tianwang Buxin Dan with modifications. For those who suffer from sunken pain in the ribs and flanks, you can use Yi Guan Jian with wolfberry and Toosendan to nourish and soothe the liver. For those with trembling fingers and tongue, add Uncaria rhynchophylla and White Peony Root to calm the liver and extinguish wind. For those with loose stools and increased frequency of bowel movements, add Atractylodes macrocephala, Coix seed, Chinese yam, and malt to strengthen the spleen and stomach. For those who suffer from tinnitus, sore waist and weak knees, add tortoise shell, mistletoe, Achyranthes bidentata and Cuscuta australis to nourish kidney yin. Long-term illness will cause damage to the vital energy, insufficient essence and blood, and cause weight loss and fatigue. Women with scanty menstruation or amenorrhea, and men with impotence can add astragalus, cornus officinalis, rehmannia, wolfberry, processed Polygonum multiflorum, etc. to replenish the vital energy and nourish the essence and blood.
