Causes

This disease is an inherited skin disease caused by abnormal keratinization process due to autosomal dominant inheritance. Some people believe that vitamin A metabolism disorders and sunlight exposure are important pathogenic factors.

Clinical manifestations

The disease usually begins between the ages of 20 and 30. The condition worsens in summer and patients are sensitive to heat. Typical sites are areas of sebum secretion, such as the face, forehead, scalp, chest and back, where small, firm, normal-skinned papules appear. Gradually, greasy, gray-brown, and black scabs cover the concave surface of the top of the papules. The papules gradually increase in size and become wart-like and merge to form irregular plaques. The proliferation of lesions caused by sweating and friction in the flexor axilla, gluteal groove and pubic region is particularly obvious, forming papilloma-like and proliferative lesions with a foul odor, covered with cracks, maceration and purulent exudate. This disease may cause palmoplantar keratosis, excessive subungual keratosis of the nails, brittle and fragmented nails, etc. In addition, the oropharyngeal, esophageal, laryngeal, and anorectal mucosa may be affected.

The characteristic pathological changes of this disease are: special morphology of dyskeratosis, forming round bodies and grains; loosening of the suprabasal spinous layer, forming suprabasal fissures and crypts; papillae covered with a single layer of basal cells, irregularly proliferating upward into the crypts and fissures; there may be papillomatous hyperplasia, acanthosis and hyperkeratosis, and chronic inflammatory infiltration of the dermis.

treat

Patients should avoid exposure to the sun. Patients with mild symptoms do not require treatment and can also use lubricants and retinoic acid preparations topically. Severe cases can be treated with systemic retinoic acid. Marriage between close relatives is prohibited.
