Pharyngeal leprosy is less common and is mostly caused by the spread of nasal tumor leprosy downward. Except for the acute edema of pharyngeal mucosa in the initial stage, it generally presents nodular infiltration, the mucosa is dry and atrophic, and nodular infiltration may occur in the uvula, hard palate, soft palate, palatine arch and tonsils. The defects and scars caused by the destruction of the lesions are often radial. The patient had absent gag reflex and open nasal speech, but no pain.

The diagnosis can be confirmed by finding Mycobacterium leprae in secretions or biopsy of lesions based on the history of contact with leprosy, chronic course of the disease, damage to the skin, mucous membranes and peripheral nerves throughout the body. Late-stage leprosy must be differentiated from tuberculosis and syphilis.

Invasion of the great auricular nerve can cause it to become thicker and larger. The facial nerve may go into spasm due to stimulation from the invasion of the lesion, or peripheral facial paralysis may occur due to compression from the lesion. The simultaneous presence of spasm and facial paralysis is a characteristic of leprosy facial paralysis.
