Maintain normal range of joint motion through passive exercise of the entire range of joint motion. It is generally recommended to do it twice a day in the early stage after the disease, 3-5 times for each joint each time, and then once a day. 2. Stretching is the most commonly used method for treating joint contracture, such as manual traction, splints, lifting platforms (such as in case of pointed feet varus), heavy objects, etc. The intensity of traction is determined by the patient's mild tolerable pain, and each traction session usually lasts about 20 minutes. Experiments have shown that short-term traction can only produce elastic extension, while repeated, especially longer-term traction can produce more plastic extension. The improvement in joint mobility (elastic extension) obtained by short-term traction often cannot be maintained, so continuous traction should be used for more severe contractures in order to achieve better results. Local heat therapy can enhance the effect of stretching. 3. Kinesiology tape corrects posture, strengthens the ankle joint and prevents Achilles tendon contracture. 4. For patients with Achilles tendon contracture, a short lower limb brace can be tried. Patients with mild Achilles tendon contracture accompanied by obvious spasm and talipes can usually recover their walking ability after using a short lower limb brace. Some patients with pointed feet can try slope-soled shoes. 5. Surgery Severe Achilles tendon contracture often requires surgery to extend the length of the Achilles tendon. Surgery can be performed for those who are refractory to the above methods.Note: 1. Perform within the physiological range of joint motion: When the joint range of motion increases and the passive joint range of motion exceeds the normal physiological range, it may cause damage and relaxation of tendons, ligaments and joint capsules, resulting in joint instability, subluxation, knee reflexes, etc. Therefore, before exercising the affected side, you should first exercise the healthy side to understand the normal range of joint motion and prevent joint contracture on the healthy side. 2. In patients with joint pain and contracture, the range of passive motion should be such that the patient feels mild tolerable pain. Pain may reflexively cause muscle contraction, making passive movement difficult. Rough exercise can cause fractures (especially in the elderly, those who are bedridden for a long time and have osteoporosis), soft tissue injuries and heterotopic ossification. 3. Passive movement should be performed slowly, because rapid movement can stimulate spasmodic contraction of antagonistic muscles and hinder movement. 4. When performing passive exercises, dorsiflexion of the foot should be emphasized. 5. When wearing a brace, you should choose a size that suits the patient to prevent long-term local compression. 6. According to the patient's condition, follow the rehabilitation plan and the guidance of professional medical staff, insist on performing ankle joint rehabilitation exercises every day, and gradually begin to use the affected limb in daily life to restore function as soon as possible. 7. When practicing by yourself, you should take protective measures such as using crutches or other objects around you to protect yourself from falling. |
